- The Great Lakes Acquisition Center (GLAC) is soliciting quotes to provide all labor, material, tools, and equipment necessary to furnish and install one (1) Boston Scientific IVUS iLab Ultrasound Imaging Cart Imaging System Model # H749ilLab 120CARTO or equal that can meet the requirements of this RFQ. Complete system required shall include the iLab System (the Cart iLab120CART), Tableside Controller Kit, Sterile Pullback Sled, Sterile Motordrive Bag, External Hard Disk, 35 GB, and Printer Paper as further defined in the specifications. All Offerors shall include two (2) copies of the equipment brochures which clearly defines the or-equal product. All services shall be performed at the Jesse Brown VAMC, Chicago IL. To the best of our knowledge, the salient characteristics of the iLab 120CARTO is the only equipment that will meet their needs however, if an Offeror can provide an ???or equal product??? the ???or equal??? product will be evaluated. The above is being issued in accordance with 10 U.S.C. 2304(c)(1) and 41 U.S.C. 253(c)(1) and in accordance with FAR 6.302-1(c), FAR 13.501(a), and FAR 8.405-6(g). NAICS: 339112 with a Size Standard is $6.5 M shall be used for this procurement. Procurement is unrestricted but all responsible sources that can meet the requirements are encouraged to participate. Specifications are available free on the Federal Business Opportunities (FedBizOpps) website: http://www.fedbizopps.gov. Any amendments to this solicitation will also be available at the URL addresses. Offerors are advised that they are responsible for obtaining amendments. Any information submitted by an offeror for consideration will not be returned and will remain the property of the GLAC. All costs associated with submitting the above are the responsibility of the Contractor and all information provided must be submitted to the Contracting Officer by the response due date and time. A signed/completed copy of the attached RFQ can be submitted electronically to the Contracting Officer at diane.brazale@va.gov as further outlined in the specification.
