Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Proposal (RFP). The solicitation number is VA-246-08-RP-0098. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-23. The NAICS is 423610 and the small business size standard is 100 employees. The Hampton VA Medical Center intends to award a firm-fixed price contract to a 100% Service-Disable Veteran-Owned Small Business (SDVOSB). This request is for purchase of Electrical Circuit Breakers to replace identified faulty breakers located at the Hampton VAMC. This requirement will require a MANDATORY site visit of all vendors interested in submitting a proposal. A site visit has been scheduled for March 3, 2008 at 9:30 AM EST. The location is VA Medical Center; Supply Building 27; Averill Drive; Hampton, VA 23667. Point of contact is Ruth B. Harris, (757) 728-3118. Offeror???s proposals shall be submitted on an offeror???s letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor. Award will be made on an all or none basis. This is a Best Value procurement. Award will be made to the offeror whose product meets the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. The following evaluation factors in descending order of importance shall be used to evaluate offers: (1) Technical; (2) Past Performance; and (3) Price. Past Performance and Technical when combined are approximately equal to price. To be considered responsible and responsive, all responses must include the following: (1) Two copies of the proposal which addresses the requirement; (2) Descriptive literature and product number for the item offered proving the product offered meets the above requirements (3) Past Performance, as follows: at least two references with contact information, for prior performance on comparable items, shall be provided who can confirm offeror is timely in delivery of items and responsive in correcting product problems; (4) Price Schedule- contractor shall submit completed price schedule, specifying pricing individual line items; and (5) Warranty Information. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.233-2, Service of Protest, 852.233-70, Protest Content, 852.233-71, Alternative Protest Procedure. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-3, Offeror Representations and Certifications-Commercial Items. In accordance with FAR 52.204-7 award will be made only to a contractor/company that is registered in the Central Contractor Registration (CCR).
