Description
PRE-SOLICITATION NOTICE: The VA National Acquisition Center will issue Request for Proposal (RFP) VA-797-08-RP-0041 for the Department of Veterans Affairs (VA) to procure Pharmacy Laser Labels. The estimated annual usage is as follows: 84,000,000 Labels per year. The offeror must supply a blue print and 100 sample labels for approval. The adhesive used on the documents must be acrylic/emulsion. The adhesive should allow for repositioning of the label without damage to the labels before becoming permanent. The offeror must ensure that the label adhesive used has been proven safe for application to plastic containers containing intravenous and irrigation solutions (examples include manufacturer premixed iv/irrigation solutions in PVC or Polyolefin bags). The paper stock must be 50-lb offset bond (20-lb bond) paper. All label material must be compatible for laser printing with an emphasis on smear and fade resistance. All adhesive label material must be resistant to damage while being stored in moist environments (example-refrigeration). The offeror will be required to keep sufficient inventory on hand to meet the needs of the VA. Orders must be shipped with 7 days of delivery. Delivery will be made F. O. B. destination to all medical centers in the U. S. and its territories (Alaska, Hawaii, Guam, Philippines, American Samoa, Puerto Rico, and the Virgin Islands). After the award the awardee is required to provide 10,000 pre- production laser label samples, 21 days after award. The contract period will be for one year plus four one-year option periods. The contract items will be distributed directly to multiple medical centers simultaneously through out the VA. If the offeror is not a manufacturer, a signed agreement between the offeror and the manufacturer must be submitted with the offer and remain in effect for the duration of the contract period including the option years. Solicitation No VA-797-08-RP-0041 will be electronically issued in commercial item format in accordance with FAR Part 12 on or about March 17, 2008, with a closing date of April 14, 2008. The NAICS code for this procurement is 322222. Any future amendments and other miscellaneous documents will be available electronically for download at the following URL: http://www.fedbizopps.gov. No paper copies of the solicitation will be available, and no telephone requests for paper copies of the solicitation will be accepted.
