Description
Description This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written quotes (oral quotes will not be accepted) on RFQ reference number VA-248-08-RQ-0216 the clause at FAR 52.211-6, Brand name or equal applies to this requirement. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-03. This procurement is being issued as a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 423390 with a small business size standard of 100 employees. This combined synopsis and solicitation is for: SUMMARY OF WORK: One time service to replace the dock leveler on the Dietary loading dock; the leveler is to have a 30,000 lb capacity and have hydraulically actuated main cylinder and lip cylinder with single button control. The lip is to extend at the top of upward travel. The leveler is to be a Serco or equivalent with a 20" lip. The electrical service is 208 Volts, 3 phases, 60 Hz. The installation is to include two new bumpers and a special deep pit conversion kit. Install a special front pit attachment kit to account for the bad concrete at the front of the pit. Add a special 12" x 74" approach plate at the rear of the leveler to cover and protect deteriorated concrete. The VA will provide a fork lift, will dispose of the old unit, and provide the electrical hook up. Ship the leveler to Bay Pines and the vendor will be notified to schedule the replacement. The price includes equipment, freight, and installation. SECURITY: The contractor and employees must comply with all security requirements imposed by the Bay Pines VAHCS while on premises. Do not leave packages? Boxes? Tool boxes etc. with out proper identification/markings SAFETY: The contractor shall comply with all Bay Pines VAHCS Safety Standards and OSHA standard of work at all times. CLEAN UP: The contractor shall leave the work area in a clean and safe condition. Removal of packing materials and de-Brea is the contractor responsibility, and disposal off Bay Pines VAHCS premises in an authorized container or landfill. EXECUTION: Performance period: Is 30 calendar days after award date. Working hours will be from 8:00 AM to 4:30 PM Monday through Friday; excluding all government holidays.
