Description
The following are vendor questions and the Government's responses: QUESTION: We have a North Carolina Home Care Agency License. We do not have Medicare Certification or Accreditation. In the bid it states that you must have "at least Medicare/State certification". Will our North Carolina Home Care Agency License suffice? ANSWER: You are eligible if you are certified and/or licensed by medicare or medicaid in North Carolina. QUESTION: We are currently accredited by The Accreditation Commission for Health Care, Inc. (ACHC). Is this accreditation accepted for this solicitation to provide Homemaker Home Health Aides to the Durham VA? ANSWER: This accreditation is pending our acceptance. A determination will be made at a later date. Please submit your proposal. QUESTION: Do we have to take every case? ANSWER: We do expect that some referrals will be rejected with appropriate justification based on specific Home Care Standards, which are also Joint Commission Standards, as listed throughout the Statement of Work (SOW) under the heading, "The Home Health Agency will . . .". Specifically, as below: 1. Initial client assessment completed within 3 working days of initial visit or as requested by patient or as requested by patient. 2. Patient Rights & Responsibilities discussed with patient. 3. Advance Directives discussed with patient. 4a. Authorization for care from patient/guardian documented. 4b. Proof that patient received copy of authorization. 5. Initial Plan of Care completed during the initial home visit. 6. Supervisory visits by RN present. 7. Supervisory visits by RN completed every 60 days. 8. Changes in the client's medical, emotional, or family condition reported to the VA contact person. 9. Plan of care revised as needed, but at least every 90 days. QUESTION: As far as the Joint Commission Standards go, what evidence to we have to show? ANSWER: An amendment will be issued addressing this. QUESTION: Are we required to meet or exceed minimum wage and fringe in the bid proposal? ANSWER: Vendor employees must be paid in accordance with the applicable DOL Wage Determination, found on page 31, at a minimum. QUESTION: Do we have to have each of our references fill out a Past Performance Survey or is this something that you would send to the references after being considered for the award? The instructions on the Past Performance Survey (pg 43) sound different than the information under Factor II-Past Performance (pg 48). Please clarify. ANSWER: Please follow the instructions on page 48 of the solicitation. QUESTION: What are the discount terms required in form 1449 block 12? ANSWER: Discount terms are the terms the vendor offers for early payment of their invoice, such as 2% net 10 days. QUESTION: What is your definition of a "Unit" on block 23 in form 1449? ANSWER: A unit is an individual, group, structure, or other entity regarded as an elementary structural or functional constituent of a whole, such as each, job, lot, issue, etc. Block 23, Unit Price, is the quantity times the price. QUESTION: Does the incumbent's (Maxim Healthcare Services, Inc.) rate include health and welfare benefits? ANSWER: No. However, the contract resulting from this solicitation incorporates the Service Contract Act, so health and welfare benefits must be paid in accordance with the applicable Department of Labor wage determination found on page 31.
