Description
The following are vendor questions and the Government's responses: 1. QUESTION: How much does maxim charge currently for a "per visit" rate? ANSWER: There is no "per visit" rate in the contract. 2. QUESTION: Does the OIG search include all agency employees or just the company as an entity? ANSWER: Please see page 6, HHS/OIG: "During the performance of this contract, the Contractor is prohibited from using any individual or business listed on the List of Excluded Individuals/Entities." 3. QUESTION: Is the maximum contract amount listed on page 6 the maximum amount that can be paid annually? ANSWER: No, it is the base plus four (4) 1-year option year total. 4. QUESTION: Does the grand total amount on page 8 represent an annual contract amount? ANSWER: No, it represents the base plus four (4) 1-year option total. 5. QUESTION: If an agency does not have 3 similar contracts as required on page 48, what will their alternative be to meet that criteria? ANSWER: Page 48, Factor II - Past Performance, states: "Offerors without a record of past performance or for whom information on past performance is not available may not be evaluated favorably or unfavorably on past performance." 6. QUESTION: Will individual client recommendations count? ANSWER: Page 48, Factor II - Past Performance, states: "In evaluating past performance, the Government may use information from . . . sources provided by prospective offerors in response to this solicitation. . . ". 7. QUESTION: Are offerors required to submit their policies on service protocols which includes admissions, discharges, assessments, supervisions, with the offer? ANSWER: No, we are interested in offerors being able to fulfill SOW requirements. 8. QUESTION: What would be considered a secondary area where an agency provides services for instance would secondary be a county that you are not licensed to cover but have an agency that you subcontract visits to in that area? ANSWER: A secondary area is an area that you have less capacity to provide services. 9. QUESTION: On form 1449 block 17 what code or facility code are we to supply there? ANSWER: Those codes will be completed by the Department of Veterans Affairs. 10. QUESTION: Would you please direct me to the where to apply for the Dun#. QUESTION: Please confirm if the RFP listed above is un-restricted. ANSWER: The solicitation is unrestricted. 12. QUESTION: Was the original solicitation un-restricted? ANSWER: Yes. 13. QUESTION: Public records indicate that Maxim Healthcare Services is a large business, can this procurement be set-aside for small business if the original procurement was set-aside for small business. ANSWER: Yes, however, the original solicitation was unrestricted. 14. QUESTION: We are seeking clarification regarding the contract services wage determination. Is the $9.69 minimum wage for Nursing Assistant I a base minimum rate required prior to the $3.16 Health and Welfare benefit, or can the $9.69 include this additional Health and Welfare benefit amount ? ANSWER: In your example above, the $9.69 does not include health and welfare benefits of $3.16.
