Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation VA-256-08-RQ-0199 is 100% set-aside for Small Businesses only. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-24. No telephone offers will be accepted. Only written offers received directly from the requester are acceptable. If submitting by FAX, you may send your offer to 318/424-6078, ATTN: Patricia J. Kim. The North American Industry Classification System (NAICS) Code is 493110. This size standard is $23.5 M. OFFERORS MUST STATE THEIR BUSINESS SIZE ON THEIR REQUEST. The base period of the award will be from date of award through September 30, 2008, with four one-year options. Overton Brooks VA Medical Center, 510 East Stoner Avenue, Shreveport, LA 71101 requires a warehouse type facility that will provide storage space for patient records that meets the following criteria: ITEM 1: 1. Initial pick up of records from current location(s) at no additional cost. 2. Provides up to 2000 square feet. 3. Climate controlled "Fire protection including sprinklers". 4. Insect and rodent controlled. 5. Twenty-four hours, seven days a week access to patient records. 6. Provide copies via fax upon request. 7. Provide pick up and delivery service available upon request. 8. Allows VA staff to pull and return records as needed at no additional charge. 9. Designated area provided for VA staff to view and purge records at all times. 10. Indexing of records for retrieval purposes and a tracking system for records removal. 11. Records placed on shelving or pallets off of the floor. 12. Storage facility is located within a 20 mile radius of Overton Brooks VA Medical Center (OBVAMC). 13. Access is limited to only authorized storage facility staff and OBVAMC staff. 14. Provide secondary security separating OBVAMC records from other agency records so no visual or physical access is allowed. 15. Storage facility staff will abide by all HIPAA Guidelines and the VA Privacy Policy. "Business Associate Agreement will be required". Base Year Price: $____________.
