- This is a combined synopsis/solicitation. Request for Quotation #RFQ VA-69D-08-RQ-0349 entitled "Certified Medical Supply Technician Services" is hereby issued for commercial items as a 100% Small Business Set-aside and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 2005-24 and VAAR 2008 Rewrite. NAICS Code 621999 applies for this procurement and the small business size standard is $9.0 million. This announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractor's proposals (cost and technical) are herein requested. A response to requirements listed below shall be submitted on contractor's letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Contractor's price and technical proposal must contain the name; title and signature of person authorized to submit proposals/bids on behalf of the contractor. Jesse Brown VA Medical Center, 820 S. Damen Avenue, Chicago, IL has determined a requirement for ONE (FTE) Certified Medical Supply Technician from April 1, 2008 through October 31, 2008. This contract requires the service of qualified, experienced and certified Medical Supply Technician who shall perform various duties and assignments in supply processing and distribution at Jesse Brown Medical Center, 820 South Damen, Chicago, Illinois. All work performed in accordance with all requirements contained in VA Hand book 7176 and in accordance with Joint Commission Standards (issued upon request). The duties will include: Tray preparation/assembly and sterilization, will receive operating room schedule and determine which cases will be needed for the following day; maintain communication with leaders and supervisors, operating coordinator and or staff. These duties will also include decontamination of reusable medical supplies and equipment that are used within the wards and clinics of the medical center and operating room suite, removal of gross contamination by disassembling and reassembling instruments and equipment where internal parts have been contaminated, and operates ultrasonic cleaner washer/decontaminator and various sterilizers. Preparation duties involve a full range of trays and sets used in the wards and operating room. Instruments will be inspected for proper functioning and gross contamination. Instruments will be sorted by size and type and trays reassembled. Packages will be inspected for sterilization. Operates a variety of sterilization equipment including plasma, steam and gas sterilizers. Items must be labeled to show date of sterilization and shelf life and stored as directed. Able to work independently with minimal supervision. Must have knowledge of aseptic techniques and wear proper work attire and personal protective equipment in designated areas. The contractor must provide the following documents for each contractor employee working under this contract: credentials and qualifications for the job; a current competence assessment checklist (an assessment of knowledge, skills, abilities and behaviors required to perform this job correctly and skillfully; a current performance evaluation supporting ability of the contractor employee to successfully perform the work required and a listing of relevant education.), certificate of liability insurance. A full-time equivalent (FTE) is defined as a contractor's employee working eight (8) hours per day, five (5) days per week (40 hours per week). Contractor's employees must be present at VA and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. A VA record-keeping system of contractor hours worked is established through time and attendance logs in the departments being served. Payments to contract personnel for any leave, including sick leave, are the responsibility of the contractor. VA is not obligated to the Contractor during technicians' absence. Evaluation of proposals offered will include the following factors: Professional qualifications, immediate availability, past performance and price. Award will be made to the contractor determined to be the "best value" to the Government. Evaluation may be made with or without discussions with the offeror(s). Contract employee must undergo fingerprinting prior to working at VA site. All responsible sources that can meet the requirements and provide services as listed above may respond to this solicitation by submitting cost and technical proposals (curriculum vitae) on their company letterhead or bid form.
