Description
This notice is not a solicitation announcement and will not constitute any commitment by the Government. A market survey is being conducted to gain knowledge of potential qualified small business sources to determine if they can effectively compete for award. The contract will be awarded with a base and four option years with an estimated value of $80,000.00 per year, totaling $400,000.00. Only Small Business firms shall respond to this notice. The U. S. Army Corps of Engineers, Mobile Dis trict anticipates future procurement requirements for Laboratory Company to provide Laboratory Services for the South Atlantic Division (SAD) covering the states of Alabama, Florida, Georgia, Mississippi, North Carolina, South Carolina and Tennessee. Serv ices will be provided to federal employees of participating agencies in accordance with the SAD Regional Occupational Health Center (ROHC), Public Health Regulations, and other directives/protocols. The Regional Occupational Health Center is located in th e Federal Building, 109 St. Joseph Street, Mobile, AL. The ROHC will provide direction for Laboratory Services within the Region. It is recommended that the Laboratory Services in the region be recruited in, or around the various District Offices: Mobi le - AL (Occupational Health Clinic - on site), Jacksonville - FL (Occupational Health Clinic- on site with periodic relief and additional nursing support services), Savannah- GA, Wilmington - NC and Charleston - SC (periodic assignment for health promotio n  no on-site clinic). Sources are being sought for firms with a North American Industry Classification System (NAICS) code of 621511. The size standard for which is a concern, including its affiliates whose average annual gross revenue does not exceed $12.5 million for the past three years. Documentation should be submitted with the following considerations: 1. Describe how your laboratory could provide same day courier service to pick up lab specimens and return reports within 24 hours to r espective physicians offices located in ENCL 1. 2. Discuss capability of providing monthly test results for employee/patient electronic charts via disk or download from a secure FTP that will interface with MEDGATE Occupational Health software. There results need to be based on HL7 v2.2 and v2.3 standards. 3. Describe your ability to establish codes for all physicians offices on the lab request form. 4. Confirm your capability of combining the lab panels that we are requesting as follows : A. Panel 1 (Standard Lab Panel) 1. Chemistry profile to include Glucose, Uric Acid, BUN; Creatinine, Serum; BUN/Creatinine ratio, Sodium, Potassium, Chloride, Calcium, Phosphorus; Protein, Serum; Osmolality (calculated); Albumin, Gl obulin, A/G ratio; Bilirubin, total; Alkaline Phosphatase, LDH, SGOT, SGPT, GGT, and T4 & TSH. 2. Lipids to include Cholesterol, total; Triglyceride, HDL, VLDL, LDL ; T. Chol/HDL ratio and Estimated Cardiac Risk. 3. Complete Blood Count to includ e WBC, RBC, Hemoglobin, Hematocrit, MCV, MCH, MCHC, Platelets, Polys, Lymphs, Monocytes, Eos, Baso, and Differential. B. Panel 1A  Standard lab panel with the addition of an Hemoglobin A1C for diabetics. C. Panel 2  Standard panel 1 lab with addition of PSA. - This panel is usually recommended for men above the age of 50 years and for men 45 years or older with a high risk for prostate cancer. D. Panel 2A  Panel 2 with addition of hemoglobin A1C for diabetics. 5. Resp onse should be submitted via email to Rosa L. Fletcher, Contract Specialist at rosa.l.fletcher@usace.army.mil.
