Description
Modification three (3) hereby extends the response date to April 11, 2008 COB. Questions to Share: PIMC PROVIDES COMMON SUPPLIES AND DISPOSABLES. IMAGES PRINTED VIA LASER PRINTER AND HAND CARRIED TO RADIOLOGIST FOR REVIEW. If those are to be supplied by the Phoenix Medical Center please advise. 1. Will IHS or the Contractor provide all disposable supplies (paper products, syringes, alcohol wipes, gloves etc?) YES 2. Will IHS or the contractor provide contrast agent? If so what number of studies require contrast agent? PIMC PROVIDES 3. Are all the completed images to be sent to a PACS system in the DICOM format? OR 4. Are all the studies to be printed on the laser printer? TO LASER PRINTER 5. What is the estimated start date of CT scanning desired in relation to the contract award date? ASAP 6. What is the number of and current skill level, certifications, experience and schooling for those "designated IHS staff" to be trained. DEPARTMENTAL STAFF CROSS-TRAINED ON THE GE zXI SINGLE SLICE SCANNER AND ARE LIMITED TO PROCEDURES OF THE HEAD, CHEST, ABDOMEN AND PELVIS; WITH OR WITHOUT CONTRAST. 7. Will the IHS provide a "crash cart" for emergency use? CRASH CART LOCATED IN DIAGNOSTIC IMAGING. CODE TEAM RESPONDS TO ALL CODES. 8. RE para G The Modular Building area is described as 30 linear sq ft x 30 linear sq ft. What is the actual linear ft dimensions of the pad? Linear sq ft is not a measurement . Are there any unusual height or other restrictions that might impede the installation of a modular building with a crane? DO NOT KNOW DIMENSIONS OF PAD. SHOULD BE OK. INSTALLATION MAY BE IMEPEDED BY USE OF CRANE. NO HEIGHT RESTRICTION, BUT BUILDING LOCATED ADJACENT TO EMERGENCY ROOM ACCESS/ROAD. 9. RE: para G Entrance canopy; Describe the estimated length of this canopy from the modular building outward. 30 FT 10. RE: para G will IHS provide access to high speed internet? YES 11. Are the estimated quantities of scans based upon actual experience at the IHS in Phoenix? YES NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (31-MAR-2008); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
