- This is a sources sought notification. Please do not submit proposals. The VA Medical Center in Coatesville, Pa. seeks a contractor to provide Inpatient Hospice Services for VA beneficiaries within the local area of the VA Medical Center in Coatesville, PA. The Hospice Agency shall develop, at the time an eligible veteran is admitted into the Hospice Inpatient Unit, a Hospice Care/treatment plan, to include the physician order, for the management and palliation of the resident's terminal illness. The Hospice Agency agrees to provide services to referred veterans according to a Hospice Care/treatment plan. Hospice Agency shall be comprised of all the services, facilities, and equipment made available to those patients otherwise enrolled in the Hospice routine program. The Hospice agency shall provide such services to residents in the home. These services shall include, but not limited to: plan of nursing, home health care aide, social worker, counseling, chaplain, and other services related to the management of the terminal illness. The Hospice Agency shall evaluate the delivery of such services through an organized, ongoing, quality assurance program. Services identified herein, shall be provided by the contractor at the Inpatient Hospice Unit. Other services may include nursing services, treatment diagnostic and preventive procedures requiring specialized skill; pain management; patient safety monitoring; scheduling of all patient visits; medication administrations; (including intravenous fluids, as needed), medicines and necessary care of the delivery lines and equipment; necessary lab-work for symptom management with reports to the physician; patient education and follow-up; teaching, supervision and counseling patient and family regarding death and dying issues; rehabilitation regiment; assessment and re-assessment of all referred patients; development; implementation, oversight and modification of patient plan of care; documentation of care and progress in patient's clinical record according to agency's established time frame; clinical pharmacy consultative and monitoring services; laboratory tests with reports to the physician; delivery of medications and supplies; patient/care-giver education in the mechanics of all equipment and procedures as necessary to reinforce in home environment.
