- (I)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, the test program in FAR Subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and written solicitation will not be issued. (II) This solicitation is issued under Request for Quote (RFQ) number VA263-08-RQ-0122. All proposals shall reference the RFQ number and shall be submitted by 4:00 P.M. Central Standard Time (CST) 1 May 2008 (III) This solicitation incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular FAC 2005-24 dated 28 Feb 2008. (IV) This solicitation is advertised under full and open competition. The North American Industry Classification System Code (NAICS) is 622110 with a size standard of 31.5M. (V) COMMERCIAL ITEM DESCRIPTION: The Government intends to award a firm fixed price contract for Infectious Disease Clinic Services to be located at the Veterans Affairs (VA) Central Iowa Healthcare System, 3600 30th St. Des Moines, IA 50310. Clinic times and days shall be agreed to by both parties. The Contractor ™s employee must be present at the VA and actually be performing the required services of this contract. The services outlined below are under the direction of the Chief of Staff or his/her designee.1.3 The Contractor shall comply with all rules, regulations and policies for the VA as well as comply with all by-laws of the Organized Medical Staff for the VA, VA regulations and Joint Commission (JC) Hospital, Behavioral Health, Homecare and Long-term Care standards, CARF and OSHA. 1.4 The Contractor ™s compensation shall be based upon the amount of time spent conducting clinics and the number of consults for inpatients and the number of Staff Education/Meetings. A logbook will be kept in the clinic and Contractor invoices shall be validated by VA personnel. 1.5 All applicable VAMC policies and procedures shall be followed by the contractor.1.6. The patients are treated at the Central Iowa VA Medical Center, located in Des Moines, Iowa. 2.0 Patient Rights 2.1 Patient privacy and confidentiality shall be maintained at all times. 2.2 Computer access will be granted on a need-to-know basis, and security of computerized information will be maintained at all times. 2.3 All patient complaints which cannot be resolved by the contractor shall be referred to the VAMC Patient Representative.2.4 - The contractor and patient care services provided shall be included in VAMC data collection activities related to patient compliments, complaints and satisfaction. 2.5 The Contractor shall be responsible for documentation of care and treatment prognosis of patients in appropriate medical charting format within their 3.5 hour clinic or within twenty-four (24) hours of an inpatient consult. 2.5.1 Documentation shall be done using the VAs ™ electronic records system and will be completed within 14 days. 2.5.2 The contractor shall fully complete all signed and informed consent documentation within VAMC-required time frames for all invasive procedures, and any non-invasive procedure requiring patient consent. (I-MED consent)2.6 Planned procedures shall not be performed without appropriate pre-procedure consent documentation.2.7 No patients shall be subject to any research activity without his fully informed consent, subject to study approval by the Investigational Research Board (IRB).2.8 The contractor shall abide by the VAMC Code of Ethics, which addresses marketing, admission/transfer/referral/discharge processes, and billing issues.3.0 Patient Safety 3.1 The VAMC shall inform the contractor of all applicable Sentinel Events or other Patient Safety Alerts received by the VAMC.3.1.1 All events related to patient injury, medication errors, and other breeches of patient safety will be reported through Patient Safety via Brief Incident Report and reported to the COTR.3.1.2 The VAMC shall provide the contractor with VAMC Sentinel Event definitions in the start-up phase.3.1.3 The contractor shall verbally notify the VAMC COTR immediately when a Sentinel Event has occurred, which is to be followed by written notification within 24 hours.3.2 The VAMC shall determine when a Root Cause Analysis (RCA) is required and when contractor is involved.3.2.1 The contractor shall participate in all Root Cause Analysis (RCAs) related to their care according to VAMC policy, and within established time frames.4.0 Patient Assessment4.1 Findings from assessment and reassessment shall be clearly documented in the medical record, which is used by all members of the interdisciplinary care team in care planning and treatment.4.2 All required documentation shall be fully completed within VAMC-required time frames of 14 days 5.0 Patient Care 5.1 All physicians performing under this contract shall be approved through the credentialing review and privileging process by the Executive Committee of the Medical Staff (ECOMS), the Chief of Staff and the Director.5.2 The Contractor shall provide qualified, experienced physicians to perform all facets of the Infectious Disease patient care during the three and one half (3.5) hour clinics and the consults. A minimum of one physician, board eligible or board certified in Infectious Disease is required. Trained and privileged backup physician coverage shall be the responsibility of the contractor. 5.3 The plan of care, all findings from patient assessment and reassessment, andtreatment provided shall be documented in the medical record, and integrated into the overall plan of care in all care settings across the continuum. 5.4 One registered nurse and one clinic clerk, provided by the VA will be on duty at all times patient care is being provided. 5.5 All medications, equipment, medical supplies, office supplies and clinic space will be provided by the VA. 5.5.1 Contractor is required to use VA formulary drugs and order all tests and labs available through Central Iowa Health Care System (CIHCS) for Lab and Radiology services.5.5.2 Transcription services are also ordered though CIHCS. 5.5.3 VA will provide access to Computerized Patient record System (CPRS) and all local computer access. Verbal, written or electronic communication between any VAMC/VISN staff and contracted staff shall not be restricted by the contractor. 6.0 Patient Education: New educational material shall be approved by the VAMC Patient Education Committee prior to the provision of patient education.7.0 Continuum of Care 7.1 The VA may evaluate the quality and professional and administrative services provided by the contractor but will retain no control over the medical, professional aspect of services rendered. 7.2 The contractor shall be responsible for coordinating evaluation, treatment, and overall medical management for eligible veterans enrolled in the clinic. 7.3 Patient care shall be monitored by the Chief of Staff via Performance Improvement Measures/EPRP for M & M review and other physician peer review processes specified to the quality of direct patient care delivered by the Contractor. 8.0 Performance Improvement 8.1 The Contractor shall participate in any Performance Improvement review or process review directly related to patient treatment or incident during his/her clinic visit. Examples of required PI activities include the VHA Performance Measures/Clinical Practice Guidelines program, and the JCAHO ORYX or Core Measurement program. 8.2 Measures related to contract compliance will be identified by the COTR.9.0 Leadership 9.1 Infectious Disease Clinic will be designed and planned by VAMC leadership staff, and will reflect a uniform level of specialty care e provided by the Contractor. 9.2 The provision of these services will be directed by the Chief Primary Specialty Medicine Service Line.9.3 The contract physician shall provide a leadership oversight role for improvement of clinical and administrative outcomes at the clinic. 10.0 Environment of Care: All significant Infectious Disease problems identified shall be reported verbally to the COTR within 24 hours.11.0 Human Resources 11.1 Outcomes from physician-specific data collection, peer review activities, and the patient compliment/complaint process shall be used in the performance improvement process, and considered at the time of re-privileging.11.2 Minimum orientation requirements shall include the following: VAMC Mission/Vision/Values, all applicable VAMC policies and procedures, Patient Rights, suspected Patient Abuse reporting, Patient Compliment/Complaint process, Safety, Patient Safety, Infection Control, Fire Safety, Confidentiality, Emergency Preparedness, incident and emergency reporting, electronic medical record training if applicable, and clinic procedures and processes.12.0 Information Management 12.1 Documentation and record keeping shall be in accordance with VAMC policies and procedures.12.2 All time frames for documentation and authentication in the medical record shall be adhered to.12.3 Verbal orders will be authenticated by the contractor within 24 hours.12.4 Contractor has the option of using the dictation system which requires a brief note at the time of the clinic and a signed dictation note within 14 days, or they may type their progress note directly into CPRS at the time of the clinic. 12.5 Identified medical record delinquency data will be forwarded to the contractor by the COTR. 13.0 Medical Staff: Contracted physician staff shall sign and agree to adhere to VAMC Medical Staff Bylaws, Rules and Regulations. 14.0 Infection Control: The Contractor will be knowledgeable of the VA Central Iowa Infection Control Plan including: Reports of surveillance, control, prevention and monitoring activities performed by the VAMC. (VII) See SOW (VIII) The following provisions apply to this solicitation: FAR 52.212-1, Instructions to Offerors, Commercial (IX) 52.212-2, Evaluation-Commercial Items. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, past performance, price and technical factors considered. The following factors shall be used to evaluate offers; Technical, Past Performance and Price. Technical and past performance, when combined, are significantly more important than price. The offeror shall refer to the Description of Requirements described in paragraph (VI) for past performance submittal requirements. 52.212-3 (Alt 1), Offerors Representations and Certifications, Commercial Items (Offerors shall include a completed copy as part of their quote or a completed registration on the ORCA website (https://orca.bpn.gov/login.aspx)). The following additional clauses and provisions are added in full text: FAR 52.217-9, Option to extend the terms of the contract states, (a) The Government may extend the term of this contract by written notice to the Contractor within 5 days; provided, that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
