- The Department of Veterans Affairs Medical Center, Omaha, NE, VISN 23, contemplates negotiation of a sole source procurement with Action Pact, Inc., 5122 W. Washington Blvd., Milwaukee, WI 53208 for the, "Tale of Transformation" training workshop. This workshop will focus on the work by Leslie A. Grant, PhD of University of Minnesota and LaVrene Norton, MSW of action Pact, In entitled, "A Stage Model of Culture Change in Nursing Facilities." These 1-day training workshop will be conducted at seven (7) VISN 23 VA Medical Center facilities (Black Hills (SD), Central Iowa Health Care System (Des Moines, IA), Fargo (ND), Minneapolis (MN), Nebraska Western Iowa Healthcare System (Omaha, NE), Sioux Falls (SD), St. Cloud (MN)). The Period of Performance for this procurement will be April 1, 2008 through September 30, 2008. The NAICS Code for this procurement is 611430 with a standard size of $6.5 million. This Notice of Intent is NOT a request for competitive proposals. However, the Government will consider all proposals received within 7 calendar days after publication of this synopsis. Interested parties may identify their interest and capability by submitting literature, detailing how their products/services meet VA requirements, to the Contracting Officer or by fax 402-449-0612. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. A determination by the Government not to compete with this proposed contract based on responses to this notice is solely within the discretion of the Government.
