- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This Combined Synopsis/Solicitation is issued as a request for quotation (RFQ) W912CZ-08-P-0115. (iii) This Combined Synopsis/Solicitation and the provisions and clauses incorporatedare those in effect through Federal Acquisition Circular (FAC) 2005-16. (iv) This acquisition is a Small Business Set-aside, Size standards $6.5 Mil. The applicable NAICS is 442210. (v) The items requested are as follows: ITEM NUMBER 1, Dining room of Building 3416-Remove existing VCT, Supply and install Mannington, Natures Path Barnwood, LVT, Supply and install 4 Burke, Gery Beige #209 Cove Base. (Government will move furniture, does not include perimeter hallway in building). RESTRICTED TO ALASKAVENDORS. A site visit will mandatory to submit a quote. The site survey and visit day will be 29 April 2008 at 1:00 pm. Be prepared to take measurements or anything else needed to determine a quote for the job. This is the only site visit that will bescheduled. The 52.212-1, Instructions to Offerors, Commercial, applies to this acquisition, along with the following addenda. (ix) Offers will be evaluated using the following evaluation factors, technically acceptable, price and delivery time. The contract will be awarded to the offeror that provides the best value to the government. (x) All offers must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications, Commercial Items (Alt I) (xi) The clause at 52.212-4, Contract terms and Conditions Commercial Items applies to this acquisition along with the following addenda: 52.219-1, Small Business Program Representations (Apr 2002). All offers must include a completed copy of this provision. 252.204-7004. Required Central Contractor Registration (Nov 2001), 252.212-7001, Contract Terms and Conditions.
