- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation number is NIHDK2008011 and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-23. The North American Industry Classification (NAICS) Code is 611430 and the business size standard is $6.5. This acquisition is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. However, this solicitation is not set aside for small business. It is the intent of the National Institutes of Health (NIH); National Institute of Child Health and Human Development (NICHD) to procure the services from First Candle/SIDS, 1314 Bedford Avenue, Suite 210, Baltimore, MD 21208 for the following service: Nurse Continuing Education Program on Sudden Infant Death Syndrome (SIDS) Risk Reduction: First Candle/SIDS Alliance is a national nonprofit health organization uniting parents, caregivers and researchers nationwide with government, business and community service groups to advance infant health and survival. With help from more than 50 local members and partner organizations, they work to increase public participation and support in the fight against infant mortality. Their mission is to promote infant health and survival during the prenatal period through two years of age through programs of advocacy, education and research, while at the same time providing compassionate grief support to those affected by an infant death. First Candle/SICS Alliance has been a co-sponsor of the NICHD-led Back to sleep campaign since inception in 1994 and has worked with the NICHD through contractual agreements to support educational outreach activities in campaign-related activities. Description of Specific Requirements to be accomplished: Task 1: First Candle/SIDS Alliance will promote the Nurse Continuing Education Program on SIDS risk reduction with an NICHD exhibit and conduct training sessions at 10 or more nursing conferences. First Candle SIDS Alliance will attend approximately 6 national nursing conferences and approximately 4 regional nursing conferences to display an NICHD exhibit and conduct sessions of the Nurse Continuing Education Program on SIDS risk reduction using NICHD materials. Task 2: First Candle/SIDS Alliance will cover registration and exhibit fees for 10 or more national and regional nursing conferences. Task 3: First Candle/SIDS will distribute and collect and NICHD evaluation form for the Nurse Continuing Education Program on SIDS risk reduction at the end of each continuing education session. Task 4: First Candle/SIDS Alliance will identify at least three hospital systems with a significant number of hospitals and approach them to see if they will be willing to adopt the Nurse Continuing Education Program on SIDS risk reduction for new baby nursery and neonatal intensive care unit nurses. The offeror must include a completed copy of the following provisions: 1) FAR Clause 52.212-1 Instructions to Offerors Commercial; 2) FAR Clause 52.212-2, Evaluation Commercial Items. As stated in FAR Clause 52.212-2 (a), The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Evaluation and Price. 3) FAR Clause 52.212-3, Offeror Representations and Certifications Commercial Items; 4) FAR Clause 52.212-4, Contract Terms and Conditions Required To Implement Statues or Executive Orders Commercial Items, Contract Terms and Conditions Commercial Items; and 5) FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items Deviation for Simplified Acquisitions. This action is under the authority of 41 U.S.C. 253(c)(1), as set forth in FAR 6.302-1 and HHSAR 306-302-1. Only one responsible source and no other supplies or services will satisfy agency requirement. Interested parties may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive quotations however; all responses received within 15 days from the date of publication of this synopsis will be considered by the Government. A determination by the Government not to compete this proposed acquisition is based upon responses to this notice and is solely for the purpose of determining whether to conduct a competitive acquisition. The offeror must include in their quotation, the unit price, the list price, shipping and handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. Note: In order to receive an award from the NIDDK; contractors must have a valid registration in the Central Contractor Registration (CCR) www.ccr.gov. Interested vendors capable of furnishing the government with the item specified in this synopsis should submit their quotation to the below address. Quotations will be due fifteen (15) calendar days from the publication date of this synopsis or by April 28, 2008, 3pm, Eastern Standard Time. The quotation must reference Solicitation number NIHDK2008011. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted in writing to the National Institute of Diabetes and Digestive and Kidney Diseases 6707 Democracy Blvd., Room 774B, Bethesda, Maryland 20892, Attention: Verne Griffin. Faxed copies will not be accepted. Emails will not be accepted.
