- Description:This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-24. Solicitation VA-251-08-RQ-0084 is issued as a Request For Quote (RFQ). The North American Industry Classification System (NAICS) Code 562111 and a Business Size Standard of $11.5 million is applicable. The Service Contract Act applies to this acquisition and the Wage Determination in effect for this acquisition is Wage Determination No. 2005-2275 (Rev 5), 03/27/2008. The Aleda E. Lutz VAMC, Saginaw, Michigan intends to solicit and award a contract to provide nonregulated waste disposal services at 1500 Weiss Street, Saginaw, Michigan. This contract will be effective from 1 June 2008- September 30, 2008 with four optional renewal years. BASE YEAR - 4 months (1 June 2008 through 30 Sep 2008) CLIN 0001 Trash Removal, 4 MO, CLIN 0002, Additional pickups, 2 EA, (OPTION YEAR ONE-1 Oct 2008 through 30 Sep 2009), CLIN 1001, Trash Removal, 12 MO, CLIN 1002, Additional pickups, 4 EA, (OPTION YEAR TWO-1 Oct 2009 through 30 Sep 2010), CLIN 2001, Trash Removal, 12 MO, CLIN 2002, Additional pickups, 4 EA, (OPTION YEAR THREE-1 Oct 2010 through 30 Sep 2011) CLIN 3001, Trash Removal, 12 MO, CLIN 3002, Additional pickups, 4 EA, (OPTION YEAR FOUR-1 Oct 2011 through 30 Sep 2012) CLIN 4001, Trash Removal, 12 MO, CLIN 4002, Additional pickups, 4 EA. BASIS OF AWARD: Award will be made on the basis of price and price alone. Statement of Work: The contractor shall furnish all supervision, equipment, labor and trash dumpster(s) for the removal of non-regulated trash from the Department of Veterans Affairs, Aleda E. Lutz VAMC, Saginaw, Michigan. Disposal shall be in accordance with local, state and federal regulations. The contractor shall provide four (4) eight (8) cubic yard dumpsters for nonhazardous trash and three (3) eight (8) cubic yard dumpsters for recyclable material. Contractor shall supply 8-cubic-yard dumpsters with hard plastic lids that close and operate properly. Non-regulated waste dumpsters shall be emptied daily, while the recycling dumpsters shall be emptied twice per week, on Tuesday and Thursday. Spills from the dumpster(s) or leakage from the truck or packer unit while on VA property shall be picked up at the time of spillage or leakage. Spills and leakage shall be cleaned up by the contractor. The contractor shall insure that trash services are accomplished daily during the hours of 7 a.m. and 1:00 p.m. The contractor shall not make any unscheduled or additional pick-ups unless specifically authorized by the Contracting Officer or the COTR. Additional pick-ups requested by the COTR shall be billed at an amount to be stated. Invoices shall be submitted in accordance with Federal Regulations 52.212-4(g) and shall be submitted to VA Financial Services Center, P.O. Box 149971, Austin, Texas 78714-8971. Remittance address: payments under this contract shall be made to the address indicated in your quote. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition with the following addendum to the provisions, delete paragraph e and h of the standard provisions. Offerors shall submit with their offer, a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. The clause at 52.212.4 Contract Terms and Conditions-Commercial Items, applies to this acquisition, to include the following addendum to the clause; 52.217-8 (which provides that the Contracting Officer may exercise the option by written notice to the contractor within 30 days), 52-217-9 Option to Extend Services; 52.232-18, 52.232-19 (which provides that funds are not authorized past September 30, of any fiscal years, unless written authorization is received from the Contracting Officer) 52.233-3, VAAR clause 853.233-70, 852.233-71, 852.237-70, 852.270-1, 852.270-4. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-34.
