- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is VA-246-08-RQ-0149 and the solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-24. This solicitation is a 100% set aside for small business. The associated NAICS code is 423450 and the small business size standard is 100 employees. The contract line items for this procurement are: 4 each - General Purpose Loop Sling with Head Support - medium (MLA2000-M); 77 each - General Purpose Loop Sling with Head Support - large (MLA2000-L); 8 each - General Purpose Loop Sling with Head Support - extra large (MLA2000-XL); 4 each - Mesh Bathing Loop Sling with Head Support - medium (MLA4060-M); 77 each - Mesh Bathing Loop Sling with Head Support - large (MLA4060-L); 8 each - Mesh Bathing Loop Sling with Head Support - extra large (MLA4060-XL); 2 each - Standard Bariatric Loop Sling Padded - large (MAA8000-L). The slings listed above must connect to an existing ARJO lift system. This is a BRAND NAME OR EQUAL requirement. "Or equal" offers must include a full product description in sufficient technical detail to allow the Government to determine the item to be at least equal to the brand name item listed above. The place of delivery is Hunter Holmes McGuire VA Medical Center, 1201 Broad Rock Boulevard, Richmond, Virginia 23249 and the FOB point is destination. The following provisions and clauses apply: FAR 52.212-1, Instructions to Offerors - Commercial - offeror will provide (1) specifications for equipment offered and (2) a bid for offered equipment, in the form of Unit Price and Extended Price for number of units specified above; FAR 52.212-2, Evaluation -- Commercial Items - award of a firm-fixed price contract will be made to the responsive, responsible offeror based on capability to meet the compatibility and timeliness (delivery) standards specified herein, at a price most advantageous to the Government; FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items - offerors shall complete only paragraph (l) of this provision if the offeror has completed the annual representations and certificates electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (b) through (k) of this provision; FAR 52.212-4, Contract Terms and Conditions -- Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items - to include items within the clause: 16,17,18,19, 20, 24, and 36. The following additional clauses apply: FAR 52.204-7, Central Contractor Registration - offerors must be registered in CCR to be considered for award; VAAR 852.203-70, Commercial advertising; VAAR 852.211-73, Brand name or equal - if bidding on an equal item, offeror must submit full descriptive literature with their bid; VAAR 852.273-70, Late offers. Offers are to be received by mail at the following address: VISN 6/CAS, Attn: Rob Betts (Bldg 27), 100 Emancipation Dr, Hampton, VA 23667 or via email at robert.betts@va.gov by May 2, 2008 at 4:30 pm (local time).
