Description
The Consolidated Patient Accounting Center (CPAC), Asheville, NC 28805 is seeking offers for a custom-built "B" Rated Double Safe, overall dimensions 62-5/8"H x 76-1/2"W x 18"D to be delivered and installed at their office at 130 Bingham Drive, Asheville, NC no later than June 12, 2008. Safe must meet the following specifications:Safe #1; 62-5/8"H x 38-1/4"W x 21-3/8"D, Outside, Burglary Class "B". Chest, 62-1/8H x 37-3/4W x 18-1/2D Inside, constructed of " thick steel with thirty (30) open areas, 6Hx12W. Doors, clear opening: 61-1/8H x 36-1/4W, Constructed of 3/8" thick steel, left door equipped with two 1" diameter chrome plated locking bolts (passive door), right door equipped with a UL listed three-wheel key-change combination lock, relocking devices, and a three-way locking mechanism with seven (7) 1" diameter chrome-plated locking bolts (active door). Lock protected by a drill-resistant hard-plate. Safe #2: 62-5/8"H x 38-1/4"W x 21-3/8"D, Outside, Burglary Class "B". Chest, 62-1/8H x 37-3/4W x 18-1/2D Inside, constructed of " thick steel with: (i) three (3) open areas, top right, 6Hx24-1/8W. and ii. Twenty-four (24) open areas, 6Hx12W. Doors, clear opening: 61-1/8H x 36-1/4W, Constructed of 3/8" thick steel, left door equipped with two 1" diameter chrome plated locking bolts (passive door), right door equipped with a UL listed three-wheel key-change combination lock, relocking devices, and a three-way locking mechanism with seven (7) 1" diameter chrome-plated locking bolts (active door). Lock protected by a drill-resistant hard-plate. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation VA-246-08-RQ-0141 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-24. No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. If requesting by FAX, you may send your request to 910-482-5226 ATTN: Melissa Garrick. The North American Industry Classification System (NAICS) number is 423420 and the business size standard is 100 employees. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. The proposed contract is 100% small business set-aside. Offers must include all costs for delivery, installation, training, and warranty. Proposals shall include specification sheets, manufacturer, and part numbers. The provisions at FAR 52.212. -1, Instructions to Offerors-Commercial Items, applied to this acquisition with the following addendum to the provisions, delete paragraph e and h of the standard provisions. 52.212-2, Evaluation-Commercial Item, the Government will award a contract resulting form this solicitation to the lowest priced, technically acceptable. The clause at 52.212.4 Contract Terms and Conditions-Commercial Items, applies to this acquisition, to include the following addendum to the clause; 52.217-8 (which provides that the Contracting Officer may exercise the option by written notice to the contractor within 30 days); 52.232-18, 52.232-19 (which provides that funds are not authorized past September 30, of any fiscal years, unless written authorization is received from the Contracting Officer) 52.233-3, VAAR clause 853.233-70, 852.233-71, 852.237-70, 852.270-1, 852.270-4. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3 Alternate I, 52.225-13, 52.232-34, 52-222-41, 52.222-42. The preceding Federal Acquisition Regulations (FAR) clauses and provisions may be accessed on the Internet at http://www.arnet.gov/far. Offerors shall submit their quotation along with the completed copy of FAR 52.212.3 Offerors Representations and Certifications-Commercial Items. Invoices shall be submitted in accordance with FAR 52.212-4 (g) and shall be submitted to VA Financial Management Service (VA FMS), PO Box 149971, Austin, Texas 78714. Remittance address: payments under this contract shall be made to the address indicated in your quote. The VA may request to see a demo at no cost to the government of any proposed machine. Offeror shall submit all technical questions regarding this solicitation to the Contracting Officer in writing, no later than April 28, 2008 at 4:00 pm EST.
