Description
DESCRIPTION/SPECIFICATIONS: Furnish temporary clerical services of one person/ forty hours per week for approximately two months at the Fort Ethan Allen Outpatient Clinic 74 Hegeman Avenue, Colchester, Vermont, 05446 for the Department of Veterans Affairs (VA) Medical and Regional Office Center, 215 North Main Street, White River Junction, VT.. NOTE: The fees covered under this contract are for professional services listed herein. The Government has no authority nor will it be obligated by this agreement to pay any additional placement fees. The following service labor shall be provided in accordance with the requirements of this contract: ServiceQuantityUnitUnit CostTotal A.Office Clerk/Typist352Hours$__________$____________ 1.POSITION DESCRIPTION/SCOPE: The purpose of this requirement is to contract one full time temporary FTE to assist with answering phones, loose filing, faxing, processing mail, checking patients in and out, etc., at the Fort Ethan Allen Clinic in Colchester, Vermont. A.Duties will include but not be limited to the following: (1)Request, acquires and assembles patient records in compliance with policy; this includes various filing activities. (2)Answers telephones and takes accurate messages. (3)Correctly checks in and checks out patients (4)Obtains and sorts mail 2.PERIOD OF PERFORMANCE: Request one individual to work, for an approximately one year, beginning April 28, 2008 thru June 27,2008. 3.CONTRACTOR REQUIREMENTS: A.General - The contractor shall provide necessary personnel capable of performing loose filing, answering phones, faxing, processing mail and checking patients in/out of clinic. B.Key Staff Qualifications - Contractor will provide individuals capable of filing loose medical documentation and/or sorting of administrative and medical documentation. Individuals must be able to comprehend and file in accordance with the Terminal Digit system, using Social Security numbers. Further individuals must under go Privacy Act training provided by the Chief of Health Information Management. As a result employees will be responsible for the protection of data and prevention of unauthorized release of information. Employees will be subject to the federal laws and regulations IAW the Privacy Act of 1974. C.HIPPA - This contract is subject to the provisions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996. D.Patient Medical Records - Contractor personnel who obtain access to medical records that may store drug or alcohol abuse data, sickle cell anemia treatment records, records or tests or treatment for or infection with HIV, medical quality assurance records, or any other sensitive information protected under 38 U.S.C. 4132 or 3305, as defined by VA, and shall not have access to the records unless absolutely necessary to perform the requirement. Any contractor-employee required to access pertinent data shall not disclose to other Contractor employees that are not involved in the performance of this task. Any violation of these provisions may involve imposition of criminal penalties. E.Individual will also - (1)have the ability to communicate with others in a manner that respects diversity and equal opportunity for all staff, patients, and visitors. (2)maintain a safe and clean environment and free of hazards (3)treat internal and external customers/patients courteously, and will be tactful and helpful in all dealings and maintain confidentiality (4)Individual will work under the direct supervision of the Clinic Manager- F.TERM OF CONTRACT - This contract period will be approximately six (6) weeks, eight (8) hours per day, forty (40) hours per week. 4.PAYMENT: Payment of sums due will be paid monthly in arrears of work performed, upon submission and receipt of a properly prepared invoice by the Contractor. The Contractor will submit a monthly bill for the hours worked, at the agreed upon price. Contractor is required to pay a minimum of the attached wage rate as listed under Section J of this contract for services performed under this contract. Payment will be authorized by the VA upon verification by the VA of the services billed and submission of documentation required under the Labor Standards Provisions of this contract. 5.WORK HOURS: The administrative working hours under this contract will be Monday through Friday, 8:00 a.m. to 4:30 p.m. with a half-hour for lunch ( 8 hours). All contract work shall be performed during these hours unless requested or approved by the Contracting Officer or COTR. The following Federal Holidays are excluded from this work schedule: New Year's Day, Washington's Birthday, Martin Luther King's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving and Christmas or any other day specifically declared by the President to be a National Holiday. No work will be performed on a Federal Holiday unless specifically authorized. 6.PERSONNEL POLICY: The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: "Worker's Compensation "Income Tax Withholding "Social Security Payments"Health Examinations "Profession Liability Insurance The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor 7.SPECIAL CONTRACT REQUIREMENTS: The contractor agrees, in accordance with the terms and conditions stated herein, to furnish qualified individuals to perform the services specified and at the prices specified in Section B of this contract. The contractor further agrees that they are the legal responsible employer and will maintain that relationship during the time its employee(s) are assigned to the Fort Ethan Allen VA Outpatient Clinic, Street, Colchester, VT., and are responsible to recruit, train, assign, pay, provide benefits and leave to, and as necessary, address performance problems, discipline, and termination of any employee employed under this contract. All payroll deductions and payment of income taxes, social security (FICA), unemployment insurance, worker's compensation liability insurance and bonding for individuals employed under this contract are the responsibility of the contractor.
