Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13 as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is DG1351-08-RQ-0450. and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC FAC 2005-24, effective on March 31, 2008. The associated North American Industrial Classification System (NAICS) code for this procurement is 315999 with a small business size standard of 500 employees. This requirement is a Small Business set-aside and only qualified offerors may submit quotations. A list of items with descriptions, quantities and units of measure are included as Attachment 1. The Department of Commerce, Bureau of Industry and Security (BIS) has a requirement for 284 escape hoods that would be used for respiratory protection in the event of a chemical or biological agent exposure. Hoods must allow a wide field of vision and a nose cup breathing interface that enables verbal communication while wearing the hood. The hood shall be a universal fit devise. See Attachment 2 for specifications of the escape hoods. Delivery is required 30 days from award. Delivery and acceptance is FOB Destination to Department of Commerce/BIS, 1401 Constitution Avenue NW, Washington, DC 20230. The following FAR clauses and provisions apply to this acquisition: FAR 52.212-1,Instructions to Offerors-Commercial Items, FAR 52.212-2, Evaluation-Commercial Items, the following factors shall be used to evaluate offers: technical capability of the item offered to meet the Government requirement and price. Award will be made to the lowest price technically acceptable, FAR 52.212-3, Offeror Representation and Certifications-Commercial Items, FAR 52.212-4, Contract Terms and Conditions-Commercial Items, FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. OFFER SUBMISSION INSTRUCTIONS-Quotations must include the following items. Submission of a quotation that does not contain all the items requested below may result in elimination from consideration for award. 1. Quotations shall include: (1)company name, address, telephone number, email address, and FAX number; (2) Solicitation number; (3) Unit price and extended price for CLIN from Attachment 1; (4) Contractor DUNS number and CAGE code, 2. Offerors are required to include a copy of the FAR provision 52.212-3-Offeror Representations and Certifications-Commerical Items with its quotation or may indicate completion of the representations and certifications on the internet at http://orca.bpn.gov.. Failure to include the certifications along with a price quotation or to complete the certification on the internet may result in elimination from consideration for award. 3. Quotations must be sent electronically in MS Word format to Pauline.Jaffe@noaa.gov and in paper format to Pauline Jaffe, Contract Specialist, National Oceanic and Atmospheric Administration, 1305 East West Highway, room 7521, SSMC4, Silver Spring, Maryland 20910, no later than 11:00 am, local time on May 5, 2008. 4.
