- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) # HHSI2462008Q0006. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-24, effective 3/31/2008. This procurement is a 100% small business set-aside under NAICS code 621399 with a standard business size of $9.0M. CLIN 0001, Contractor shall provide a firm, fixed-price hourly rate (all inclusive to include travel and per diem) for 864 hours, May 9, 2008 to May 8, 2009. Description of Requirements: Contractor shall provide a part-time contract pharmacist whose mission is to provide accessible, full-scope, quality pharmacy services to eligible Indian patients at the Wewoka Indian Health Center. The services will include the following: 1. Assure appropriateness of drug therapy. 2. Verify that patients understand their medications and appropriate outcomes of their drug therapy. 3. Assure availability, preparation and control of medications. 4. Provide drug information, drug therapy consultation and staff education relating to drug therapy. 5. Provide health promotion and disease prevention activities relating to drug use and preventive drug therapy. 6. Manage therapy for selected patients in whom drugs are the primary method of treatment. 7. Performance of other assigned duties or agency-specific activities including administrative duties. 8. Performs a variety of professional tasks of average technical difficulty in fulfillment of the department’s basic patient care and service functions. 9. Exercises independent judgment in accomplishment of a variety of professional tasks. Judgmental failure could seriously affect the health status of one or more individuals. 10. Participate in a quality improvement program consistent with current Joint Commission standards for ongoing monitoring and evaluation of the quality and appropriateness of care. Qualifications •Hold a current, unrestricted state professional license in pharmacy from any of the 50 United States or the Commonwealth of Puerto Rico. •Notify the WIHC of any changes in contract privileges at any non-IHS facilities. •Have no work or health restrictions interfering with the performance of pharmacy services. •Speak, understand, read, and write English fluently. Communicate with sufficient clarity to be understood by the patients and health staff of the WIHC. •Possess sufficient initiative, interpersonal relationship skills, and culturally sensitivity such that he/she can relate constructively to patients from the Native American community. •Standards of personal hygiene and grooming such that his/her image as a pharmacist is compatible with the expectation of the WIHC staff and the Indian community. •Indemnify the Government for any liability producing act or omission of the Contractor, its employees and agents occurring during contract performance. The Contractor shall provide 3-4, 8 hour, days of pharmacy services per week. Services shall be scheduled in a manner that is mutually agreed upon between the Contractor and the service unit. This is a nonpersonal health care services contract. The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical, professional aspects of services rendered (e.g. professional judgement, diagnosis for specific medical treatment). Government Furnished Property, Facilities and Services •Administrative and professional support that is available to Indian Health Service personnel. This includes but not limited to copy machines, telefax machines, medical library, and telephone services. •Initial and ongoing orientation to the WIHC Service Unit, clinic policy and procedures related to performance of duties outlined in the description of requirements. •Use of government furnished facility space, equipment, supplies, and personnel are for the provision of care and services set forth in the contract only. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: (1) completed “Temporary Waiver of Character Investigation” and “Declaration for Federal Employment” form (2) verification of indemnification (3) three past performance references to include the contact name and phone number, contract number, company name, brief description of project; (4) Dun and Bradstreet Number (note: Contractors must be registered in the Central Contractor Registration to be eligible for award. This can be done at http://www.ccr.gov). The provisions of 52.212-2, Evaluation – Commercial Items (Jan 1999) applies to this acquisition. The Government intends to award three purchase orders resulting from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Price; (2) Qualifications (3) Past Performance and (4) compliance with Indian Child Protection requirements. All factors are equal in importance.
