Description
The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, Texas, intends to make a single award to a responsible quoter for the provision of (1) one Optometrist at the Federal Correctional Institution located in Bennettsville, South Carolina 29512. The North American Industry Classification System (NAICS) Code is 621320 (Size Standard: $6.5 million). All duties shall be performed in accordance with standards and methods generally accepted within the field of Optometry services, and in compliance with all Federal and State policy and regulations. The contractor(s) performance shall be monitored by the COTR and the contractor(s) must have the ability to work with an inmate population from a variety of cultural, regional, racial and ethnic backgrounds. The contractor(s) shall at a minimum have 18 months of experience as a Optometrist, and be a graduate of an accredited school of Optometry. Finally, the contractor must possess and maintain a current license and registration for the state of South Carolina to practice Optometry without restrictions. The candidate(s) name, resume and photocopies of documents evidencing education and training must be submitted with each quotation. It is estimated that the contractor will provide one (1) Optometrist to perform an estimated 312 one (1) hour sessions per year. A session is defined as one (1) hour. The contract period will consist of a 12-month base period and four 12-month option periods for renewal. The solicitation will be available on or about June 5, 2008, and will be distributed solely through the website at ww.fedbizopps.gov. All future information concerning the acquisition, including solicitation amendments will be distributed through the website. The anticipated receipt of quotes is July 1, 2008. The solicitation is restricted to small business sources. All responsible small business sources may submit a quotation which will be considered. Faith-Based and Community-Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. Please note: All contractors doing business with the Federal Government must be registered in the Central Contractor Registration (CCR) database. The website for registration is www.ccr.gov.
