- This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 0277075 and the solicitation is issued as Request for Proposal (RFP). This solicitation is for the procurement of a one (1) Superdome Uplift Kit and three (3) Superdomes (See attached RFP). A description of the requirements for the items to be acquired is included in the attachment. The delivery of the Uplift Kit is requested 14 days after the contract award date and 30 days after contract award for the remaining Superdomes. The provisions at 52.212.1, Instructions to Offerors --Commercial, applies to this acquisition. North American Industry Classification System (NAICS) code and small business size standard. The NAICS code is 334111. The small business size standard is 1,000 employees. Standard Form 33 will be submitted with the proposal (SF 1449 does not have to be submitted with the proposal). The provision at 52.212-2 Evaluation -- Commercial Items follows: 1. Each Offeror is requested to submit a quote to include unit price, lead time, and total price (See attached RFP). 2. For each item purchased, a Firm Fixed Price Offer shall include 16 months of Critical Care Maintenance Support. 3. Basis for award will be combined total of items requested. 4. Provide delivery date. 5. The proposal evaluation will be based on an integrated assessment of the information submitted in the Offeror's proposal and other information available to the Government. The integrated assessment will address the ability to meet the technical requirements as outlined in Section C- Requirements, past performance, and cost. All elements are considered of equal importance. The provisions at 52.212-3, Offeror Representations and Certifications- Commercial Items, applies to this acquisition. An Offeror shall complete only paragraph (l) of this provision if the Offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the Offeror shall complete only paragraphs (b) through (k) of this provision.
