Description
VA Network 22 Logistics Office, Long Beach, California intends to negotiate a commercial FAR (Part 12) Sole source procurement with Quality Assurance Services, Inc, 1500 Via Hacienda, Chula Vista, CA 91913. This procurement is being conducted in accordance with FAR 6.302-1-only one responsible source and no other supplies or services will satisfy agency requirements, incorporates provisions and clauses as those in effect through Federal Acquisition Circular (FAC) 2005-25. A justification for other than full and open competition has been prepared in support of this acquisition. This BOS announcement constitutes the only solicitation issued; a separate written solicitation shall NOT be issued. Request for Quotation is RFQ VA-262-08-RQ-0242IM. The North American Industry Classification System (NAICS) Code is 811219 and the small business size standard is considered 6.5M. This procurement is for Compliance and Radiation Safety Survey for the VA Greater Los Angeles Healthcare System, includes the VA Ambulatory Care Outpatient Clinic, at the Sepulveda (North Hills), at the VA Bakersfield, and at the VA Santa Barbara. The Contractor shall provide Radiation Safety Surveys and Medical Physics Analyses of equipment that produce ionizing radiation for the Department of Veterans Affairs and furnish a written report of surveys within five (5) working days of any site visits. These surveys shall be conducted to comply with the Joint Commission on Accreditation of Health Care Organizations (JCAHO), Title 21, U.S. Department of Health and Human Services (HHS), FDA and The American College of Radiology (ACR). Period of performance for services shall be from July 1, 2007 through July 31, 2008 with the final report due no later than August 31, 2008. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE QUOTES. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. The Government shall consider all proposals received by 4:00 PM Pacific Standard Time May 28, 2008. Information received shall be considered solely for the purpose of determining whether to conduct a competitive procurement. No solicitation document exists and no telephone inquiries shall be accepted.
