Description
The contractor must be accredited by a nationally recognized accrediting body (i.e. ACHC/CHAP/TJC) according to Home IV Therapy Standards, Medicare certified and licensed by the state/states in which service is provided and remain in compliance throughout the duration of the contract. Proof of such accreditation/licensure must be submitted to the VA initially and on an annual basis. The Home IV Therapy agency shall provide services according to their own agency policies and procedures which will be in compliance with a nationally recognized accrediting body, Medicare Conditions of participation and applicable state rules and regulations. Services provided under this contract shall meet all current Home Care and national accrediting body standards, Center for Disease Control (CDC) guidelines and standards for equipment management and Federal Drug Administration (FDA) requirements. In addition, Contractor shall comply with all Department of Transportation and FDA regulations. Failure to remain accredited (if already accredited) or to meet the standards may be grounds for termination of this contract. The contractor shall furnish the management, supervision and personnel necessary to provide all Home IV Therapy Services including antibiotic, antiviral, and antifungal therapy, as authorized by VA, to VA patients in their homes or other choice of residence within the service area of the NWI VA area of responsibility. Contractor shall provide: 1. All medication, supplies, equipment, and infusion solutions, pharmacy management including pharmacy compounding using aseptic techniques in a laminar flow hood when applicable (first dose of a medication shall be administered in a supervised care setting unless otherwise specified). 2.Delivery, assembly and set-up of infusion equipment by trained personnel in patient's home, or in an area of the patient's preference. Delivery of equipment shall meet all applicable Federal, State and local laws. 3. Delivery, inventory and re-supply of all ancillary medical supplies required for infusion therapy. 4. IV medication administration by a qualified healthcare professional in accordance with nationally recognized accrediting body standards. Contractor's initial patient assessment may be conducted at the VA, where the Contractor shall meet patient's family and begin the patient/caregiver education program. Contractor's healthcare professional shall be responsible for continual patient reassessment and additional education, as required. 5. Training of patient/caregiver, by a qualified healthcare professional (i.e. registered nurse or pharmacist), in the skills required for self-administration of infusion therapy, in accordance with nationally recognized accrediting body standards. Contractor's initial patient assessment may be conducted at the VA, where the Contractor shall meet patient's family and begin the patient/caregiver education program. Contractor's healthcare professional shall be responsible for continual patient reassessment and additional education, as required. 6. Training of the patient/caregiver in the proper use and monitoring of infusion equipment. Contractor personnel who are familiar and experienced with the type of equipment being utilized shall conduct training. All training shall be provided in a courteous and simplified manner, which can be easily understood by the patient/caregiver. 7. Training patient/caregiver in the proper use of medical supplies, by qualified contractor personnel, and instruction in the safe disposal of medical supply waste. Contractor shall be responsible for ensuring patient/caregiver maintains proper storage conditions for medications, equipment and supplies, i.e. space, cleanliness, temperatures and disposal of supplies. 8. Routine laboratory tests, drawn as ordered, in accordance with standard medical practice for the disease and medication indicated.9. Patient monitoring and all additional follow-up care required by the patient. This shall include coordination, supervision, evaluation and assessment of the appropriateness of care. 10.Maintenance of any necessary parenteral access devices, which shall include central line de-clotting/repair. 11. Nutrition management/dietetic monitoring for any client on home TPN shall be conducted by a dietitian or other qualified healthcare professional. 12. Maintenance of equipment under a preventative maintenance plan that includes electrical safety. 13. Clinical pharmacy consultative service. 14. A 1-800 number available 24 - hours a day, 365 days per year, for patient/caregivers to call for problems or questions that arise during the course of the home therapy. Patients/caregivers shall receive assistance and answers within 2 hours of making initial contact with the Contractor. Assistance shall be provided by a qualified, appropriate, health care professional. The Contractor shall notify the Community Health Nurse Coordinator (CHNC) VA Administrative Officer of the Day within 24 hours of any problems patients encounter with their home therapy. VA Telephone Care 866-687-7382 is available seven-(7) days per week twenty-four (24) hours per day. 15. Twenty-four hours, 365 days per year, pharmaceutical and nursing coverage for the patient, including emergency service supply of replacement/back-up equipment and/or supplies if necessary. 16. All emergency or on-call services will be performed without compensation from the VA. 17. Hazardous waste disposal. Contractor shall remove and dispose of bio-hazardous waste from patients' homes as recommended by manufacturer and applicable local, state, and federal regulations. Care shall be provided regardless of race, color, religion, sex, or national origin. The VA staff will obtain services from an agency other than the contracted agency, when, by their professional assessment, the other agency would be the better resource to meet the needs of the patient or family. The overall objective is to provide timely and top quality Home IV Therapy Care for authorized veterans in Nebraska Western Iowa area, reduce the cost to the VA, and gain efficiencies in managing the Home IV Therapy program.
