Description
The Centers for Medicare and Medicaid Services (CMS), Office of Acquisitions and Grants is seeking capability statements from businesses, in particular Small Business sources, (e.g., 8(a), service-disabled veteran owned small business, HUBZone small business, small disadvantaged business, veteran-owned small business, and women-owned small business) who are qualified to provide composition support (such as desktop publishing) for CMS publications in a large print format (English and Spanish). A detailed Statement of Work for the composition support tasks has been attached. Desktop publishing work involves formatting, revision, proofing print products, quality control, creating Section 508 deliverables, and final deliverables. The Centers for Medicare and Medicaid Services must fulfill its statutory mandate to provide low vision Medicare beneficiaries with information about their Medicare benefits, choosing a health plan and a prescription drug plan, and Medicare privacy rights. In addition, this is in accordance with providing low vision Medicare beneficiaries with equal and timely access to Medicare health insurance information, in compliance with the reasonable accommodation provision of the Americans with Disabilities Act, June 1990. The contract actions associated with this procurement will be for a period of performance of 12 months. At a minimum, your capability statement shall include the following information: 1. Business Information: a. DUNS: b. Company Name; c. Company Address; d. Type of Company (i.e. small business, 8(a) woman owned, veteran owned, etc.) As validated via the CCR. All offerors must be registered in the Central Contractor Registration located at www.ccr.gov. e.Company Point of Contact and Phone. Responses must be submitted electronically to the contract specialist listed below no later than June 9, 2008. Capability statements will not be returned and will not be accepted after the due date. The maximum number of pages for submission is 10 pages. This Sources Sought Notice is for information and planning purposes only and is not to be construed as a commitment by the Government.
