Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, F3H3G3129A002, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-21. This procurement is a 100% Total Small Business set a-side IAW FAR 19.502-2 (a). The NAICS code is 235510 and the small business size standard is 500 employees. The following commercial items are requested in this solicitation; Statement of Work Refinish Floors building 456 rm. 113 1. Scope: Refinish floors in room 113 of building 456. Vinyl tile will already be removed before start of work. Area to be finished is approximately two thousand and fifty (2,050) square feet. Install approximately one hundred and seventy (170) lineal feet of four inch gray Flexco VCB-036 vinyl cove base. Install 11 lineal feet of 1/2" epoxy sloping at door. 2. Major Items of Work: a. Finish concrete with an epoxy type coating. Product used should be ECO-PT250 Resurfacer Flooring System with a ECO-HTS 100 Chemical Resistant Urethane Top Coat (or equal). b. Finish shall be a smooth satin finish. c. Remove vinyl tile adhesive, pre-fill any cracks, holes and other imperfections d. Apply product in accordance with manufacturer's written instructions. 3. Location of Work: All work will be performed at Building 456 room 113 located on Offutt AFB, NE. 4. Codes: The work shall conform to the latest applicable Unified Facilities Criteria (UFC), and in accordance with the State, Local and Federal Government Laws, Regulations and Building Codes. Contractor is responsible for all field measurements for proper material installation. 5. Occupancy: During construction the Government will occupy the existing facilities. The contractor shall not restrict nearby pedestrian or vehicular traffic to this facility and shall coordinate construction with the government representative to minimize conflicts and to facilitate usage. The contractor shall perform the work so as not to interfere with ongoing operations. 6. Utility/Service Interruptions: The government representative shall be notified not less than 15 calendar days in advance of any interruptions of building utilities/services. The contractor shall describe in writing the work to be done and the time and duration of the interruption. No work shall be done until approval is received from the government representative. 7. Clean Up: The contractor shall assure that the work area is cleaned up at the end of each day. Debris and trash shall be removed and properly disposed of by the contractor outside the confines of Offutt AFB. 8. Work Schedule: The working hours for this contract shall be between 0730 and 1630 hours daily, excluding Saturdays, Sundays, and Federal Holidays. 9. Performance Period: The suggested performance period will be 16 June, 2008 to 27 June, 2008. Work is required to be completed by 30 September 2008. 10. Use of Utilities: All reasonable quantities of utilities will be made available to the contractor without charge. Any temporary connections of lines that may be required will be installed, maintained and removed by the contractor at his own expense and in a manner satisfactory to the government representative. Removal of such connections or lines shall be accomplished prior to final acceptance of the contract. 11. Guarantee: The materials and craftmanship furnished under this contract shall be guaranteed against defective materials, design, and workmanship for the full warranty time which is standard with the manufacturer and/or supplier, but in no case less than one year from the date of acceptance either for beneficial use or final acceptance (whichever is earlier). Upon receipt of notice from the Government of a failure of any part of the guaranteed equipment covered by the scope of the guarantee, during the guarantee period, any defective work or materials shall be replaced promptly by workers skilled in that particular trade, at no additional cost to the Government. 12. Inspections: The Government representatives will perform as a minimum the following progress inspections: a. Start up b. Preparation/Removal c. Final 13. Associated Costs: All of the costs in connection with this contract shall be included in the lump sum price for all work to be performed under this contract. A site visit will be conducted on Thursday the 5th of June, 2008 at 2:00 pm. Pick up will be in the parking lot of Rummors off of Fort Crook RD at 1:45 pm. All interested vendors must submitt any questions and number in party to SSgt Dustin Hoffmann no later than the Wednesday 4th of June. Vendors are asked to limited their party to two people due to limited transportation space.
