Description
This is a market survey to gain knowledge of potential qualified sources to provide medical services to include applicant and incumbent medical examinations, fitness for duty examinations, medical consultations, and data management and medical standards validation. These services shall be provided in accordance with all applicable Federal, State and local laws. These may include, but are not limited to: Clinical Laboratory Improvement Act (CLIA); Occupational Safety and Health Administration (OSHA) and all state OSHA programs. In addition, Title 5 of the Code of Federal Regulations, U.S. Department of Commerce Office of Management & Organization Departmental Administrative Order Number DAO 202-958 and NOAA/NMFS specific requirements also provide compliance standards. These services will be required at all National Marine Fisheries Services Office for Law Enforcement locations throughout the U.S. where applicants are selected for Law Enforcement positions. The Office for Law Enforcement is organized into a Headquarters in Silver Spring, Maryland and six operational Field Divisions located in Gloucester, Massachusetts; St. Petersburg, Florida; Long Beach, California; Seattle, Washington; Juneau, Alaska; and Honolulu, Hawaii. The number of examinations to be performed on an annual basis depends upon the number of law enforcement personnel currently on staff as well as new applicants. In addition to Medical Examinations and Fitness for Duty Examinations, Occupational Medicine Physical Consultations (includes expert witness; subject matter experts); Psychiatrist Consult; Psychologist Consult; Medical Standards Validation, Occupational Medicine Physician, Audiologist, Optometrist and other servivces as needed will be required. The number of examinations varies, but range between 150-300 exams per year. Interested vendors may identify their interest and submit written documentation that demonstrates their ability to provide these servivces by responding to this notice by 2:00 p.m. June 18, 2008. This notice is NOT a request for proposals. Capability statements received will be considered. These statements must provide the company's experience in terms of time in business, number of clients served, number of locations, capacity to perform the required services, quality and quantity of personnel, and a price schedule for the services anticipated to be required.
