- STATEMENT OF WORK PROJECT XHZG200800722 REPAIR BASE BACKFLOW PREVENTERS Tulsa, OK, OK Air National Guard GENERAL REQUIREMENT: SCOPE: The contractor shall provide the necessary personnel, equipment, and materials required to repair base backflow preventers. All work performed under this Statement of Work (SOW) shall be conducted in accordance with all applicable state and federal laws and regulations, ANG policies and regulations, Base MATOC Specifications and this SOW as outlined below. Proposal shall include site visits, as required, compilation of required data, and technical analysis. Work will be performed Monday Thursday 7:00- 5:30 and must be coordinated work thru the BCE. 1.The work to be performed is listed as follows: REPAIR BASE BACKFLOW PREVENTERS IN THE FOLLOWING BLDGS. 230, 240,304,313,314,316,318,320,329,330,403,501 1.0 Scope 1.1 The contractor shall provide the necessary personnel, equipment, and materials required to repair base backflow preventers that were found during base backflow inspection (See attached AF Forms 845s). All work performed under this Statement of Work (SOW) shall be conducted in accordance with all applicable state and federal laws and regulations, ANG policies and regulations, and this SOW as outlined below. 1.2 The tasks of the SOW are: * Bldg. 230-2 Provide an approved 4 inch air gap on relief valve. * Bldg. 240-1 Remove extension on No.1 test cock, and relocate to appropriate installation requirements. * Bldg. 240-2 Remove bypass and relocate device to appropriate installation requirement. * Bldg. 240-5 Remove bypass and relocate device to appropriate installation requirement. * Bldg. 240-13 Install backflow preventer * Bldg. 304-1 Install an approved 1 inch air gap * Bldg. 313-1 Relocate device to appropriate installation requirement * Bldg.314-1 Relocate device to appropriate installation requirement * Bldg. 316-4 Install Backflow preventer * Bldg. 316-8 Relocate device to appropriate installation requirement * Bldg. 318-2 Install backflow preventer * Bldg. 320-1 Relocate device to appropriate installation requirement * Bldg. 320-5 Relocate device to appropriate installation requirement * Bldg. 329-1 Install an approved 1 inch air gap * Bldg. 330-2 Relocate device to appropriate installation requirement * Bldg. 403-3 Install an approved 4 inch air gap * Bldg. 501-3 Lower pipe to appropriate installation requirement and install backflow preventer * Bldg. 501-9 Install backflow preventer Note: The maximum height of backflow preventers installed / relocated will not exceed four ft. 2.0Contractor's Fire Protection: Contractor shall provide his own fire protection to include fire extinguishers. Open flame devices or arc welding is not permitted except when a permit has been applied for and received. Permits shall be obtained from the Base Fire Department, Building No. 403, and are required 72 hours prior to using any open flame devices or welding devices on base property. 3.0 Hazardous Material Usage: The contractor planning to use hazardous material (HM) on base for the work outlined above shall register with the base Hazardous Material Pharmacy (HMP) prior to start of work in order to support the installations compliance with Executive Order 12856, Federal Compliance with Right-to-Know Laws and Pollution Prevention Requirements. All HM brought on base that is required to support this contract shall be reported to the HMP using the Contractor HM Identification Form. The Contractor HM Identification Form shall be provided to the contractor at/or prior to the Pre-Construction meeting. (See Attachment) 4.0 Damage to Government Property: The contractor shall immediately notify the government project manager if any Government property is damaged by the Contractors operations and immediately repair or replace the damaged item to match the existing at no additional cost. 5.0 Performance Period: 30 Days 6.0 Contractors Equipment & Supplies: Store all Contractor tools, equipment and supplies in a location approved by the government project manager, on or off base property. Government shall not be held responsible for missing or damaged contractor owned equipment or materials. 7.0 Trash, Refuse & Debris: The contractor shall not use any government disposal bins, dumpsters or trash receptacles for any reason at any time. The contractor shall provide appropriate disposal bins/trash receptacles. Approval of disposal bins/trash receptacles location(s) shall be coordinated through government project manager. The contractor shall not allow debris to leave the work area due to potential damage to aircraft engines. Contractor shall coordinate with the base Environmental Management Office (EMO) prior to disposing any materials off base. All debris or trash shall be disposed of off base. All demolition debris shall be disposed of at a permitted landfill, with landfill authorization, unless other arrangements have been coordinated through the EMO. Provide proof of landfill disposal from the landfill that accepts the materials. This may be accomplished by providing the government project manager with landfill receipts; waste manifests, or weight tickets from the landfill that accepts the construction debris. 8.0 Material & Equipment Accountability: Notify the government project manager prior to delivery of any material or equipment to the site. The contractor shall ensure someone is available to receive delivered materials and able to store them safely in an area to be coordinated with the government project manager. Any materials delivered without prior coordination and without having someone to take receipt, shall not be accepted and turned away. 9.0 Prior to any work the contractor shall submit literature on and AF form 3000 for all products to be used, manufacturers installation instructions. Operations and maintenance manuals shall be provided at the completion of the job for newly installed equipment and provide equipment and provide material safety data sheets for all products. 10.0 Call Back: The contractor shall be required to accomplish additional services, at no additional cost to the government, following an initial service which has not provided the service required.
