- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. RFQ# W44DVQ80817701A is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. This requirement will be processed under Commercial Item Procedures IAW FAR Part 12. This procurement is unrestricted and all responsible sources may submit a quotation, which will be considered. The requirements in this solicitation are: Clin0001, 1ea #Dicom Express-001 CD/DVD Burner Package to include: Dicom Express Rimage Disclab 5200 with 2 CD/DVD Drives & Everest Thermal Printer; PACStogo Pro Dicom part 10 software with one year free warranty & all upgrades; Support for intelligent multiple-disk spanning; Support for intelligent multiple-disk spanning; Dell PC, Flat Screen Monitor, Keyboard; 1styr Software & Hardware maintenance w/Remote support; Media (100) White Thermal CDs; Custom Label design for cd/dvds; Custom Auto CD Load Splash Screen; On site installation & training; Rimage Rapid Exchange Replacement Warranty 1st year. This system is an accessory to the existing DINPACS System at Ft Sill. The salient characteristics are as follows: Workstation must be networked to an existing Picture Archiving & Communications System (PACS); must be able to send, receive and store dicom studies; must be able to create a CD with custom labels and print patient and examination information; must have the E-Film Lite Software; must be able to receive and place images on one or more cds/dvds automatically from any other workstation on existing PACS network; must have a DICOM Compliance Statement; must automatically generate a DICOMDIR root file, stored on each produced cd/dvd; must be compatible with all DICOM-compliant PACS, multiple filters; must be able to connect to existing PACS network; must be able to produce 40 plus cd/dvds per hour; and must have a web interface to allow one to perform all necessary functions to create and burn a CD without physically being at your CD burner workstation. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, within 30 days after receipt of order, shipping shall be FOB destination to Reynolds Army Community Hospital, Bldg 4300 Thomas Rd, Fort Sill, OK 73503-6300. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Technical Capability (compliance with the requirements of FAR Clause 52.211-6 if vendor offers and Equal Product) and Price, with Technical Capability being significantly more important than Price. Vendors shall include a completed copy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation. FAR clause 52.212-4 [Contract Terms and Conditions Commercial Items] applies to this acquisition to include the following addenda: Defense Federal Acquisition Regulation Supplement (DFARS) 252.232-7003 Electronic Submission of Payment Requests and DFARS 252.211-7003.
