Description
Revised drawings and specs (complete new sets of each) were issued 6/9/08 and will be available at no cost in paper or CD form from Plan & Print Systems, 6295 East Malloy Road, East Syracuse, NY 13057, (315) 437-5111. They will also be available for review at Syracuse Builders Exchange, 6563 Ridings Road (315) 437-9936. The following is included in Amendment #1 to the Solicitation that is part of the revised specs. THE FOLLOWING IS APPLICABLE ONLY TO OFFERS FROM OTHER THAN SMALL BUSINESS: In accordance with FAR 52.219-9, a subcontracting plan is required to be approved prior to award of any contract resulting from this solicitation if that award is to be made to OTHER THAN small business. Any Offeror completing FAR 52.219-1 certifying that the Offeror is NOT a small business is required, as part of its offer, to submit a draft subcontracting plan. The completeness and acceptability of this plan will be used as part of this evaluation factor for any such Offeror. THIS PLAN SHOULD BE SUBMITTED AS AN ATTACHMENT TO THE COST PROPOSAL. Eric BurnhamPower Comm Electric Jeff ShermanSherman Construction The following Project Narratives were reviewed at the June 4th Pre-Proposal Meeting: Item 1 (Project 1): 528A7-08-702, Renovate Imaging for Angio & CT New Equipment A.Location: 3rd Floor West B.Size: Approximately 2,600 SF (Angio) + 350 SF (CT) C.Contractor Scope of Work for Angio: Removals, reconfigurations and reconstruction to provide a new Angio Suite. Work includes: 1.Removal of asbestos-containing floor tile and pipe insulation 2.Replacement of walls, cabinetry, doors, frames, hardware, floor finishes, wall finishes, and ceilings 3.Replacement of HVAC ductwork, diffusers, radiant panels, piping, and valves 4.Replacement of plumbing fixtures, piping, and valves 5.Replacement of sprinkler heads and piping 6.Replacement of electrical systems including lighting, power, smoke & fire detection, communications, and radiology electrical systems D.Contractor Scope of Work for CT: Removals and replacement of some of the radiology electrical system; minor ceilings modifications to accommodate new equipment E.Work by Others: 1.Removal of existing Angio & CT equipment 2.Installation of new Angio & CT equipment 3.Removal and reinstallation of existing loose furnishings F.Key Issues: Close coordination is required for all trades to accommodate new construction above ceiling and below floor of Angio & CT rooms Item 2 (Project 2): 528A7-08-709, Renovate 5 South for Outpatient PT A.Location: 5th Floor South B.Size: Approximately 6,900 SF C.Contractor Scope of Work: Removals, reconfiguration, and reconstruction to provide a PT Suite, including therapy rooms, toilet rooms, storage rooms, exam rooms, offices, nurse's station, waiting areas, etc. Work includes: 1.Removal of asbestos - containing floor tile and pipe insulation 2.Replacement of walls, doors, frames, hardware, floor finishes, wall finishes, and ceilings 3.New cabinetry and millwork 4.Replacement of HVAC ductwork, diffusers, radiant panels, piping, and valves 5.Replacement of plumbing fixtures, piping, and valves 6.Replacement of sprinkler heads, and piping 7.Replacement of electrical systems including lighting, power, smoke & fire detection, and communications D.Key Issues: The entire project area is over the existing operating rooms suite; operating room access is limited E.Work by Others: 1.Removal/reinstallation of loose furnishings and equipment Item 3 (Project 3): 528A7-08-704, Renovate for Low Vision A.Location: 2nd Floor South B.Size: Approximately 1,200 SF C.Contractor Scope of Work: Removals, reconfigurations, and reconstruction to provide a new low Vision Suite.
