Description
The VA Boston Healthcare System is seeking a qualified a/e to develop complete construction drawings, specifications and cost estimates for project 523-370, Renovate Eye Clinic (JP) at the Jamaica Plain Campus located at 150 South Huntington Avenue, Jamaica Plain, MA.Ophthalmology Section, Optometry Section, Advanced Low Vision Clinic of Surgical Svc, VIST (Visual Impairment Svc Team Blind Rehab) ~20,000 NUSF space will be renovated to improve patient flow, environment and infrastructure 1) Provide upgrade HVAC, electrical, and plumbing systems 2) Upgrade architectural finishes and correct a severely deficient functional layout 3) Using the VA Space Criteria as a guide, meet the needs of Ophthalmology, Optometry, Advanced Low Vision Clinic and VIST (Visual Impairment Svc Team Blind Rehab) as delineated during preliminary project meetings and site surveys Primary focus shall be the complete design of Ophthalmology Section space on Wards 7A, 7C, 7D, 8C using the VA Space Criteria as a guide including upgrading architectural, plumbing, HVAC, electrical systems to improve patient flow, environment and infrastructure. It will correct a severely deficient functional layout that precludes proper visual surveillance of patients, eliminate inappropriate separation of the medication preparation area, incorporate the patient waiting area, Eye Exam Lanes and ancillary support areas to provide proper patient safety as well as a more efficient working environment. It will accommodate the co-location of office/exam space to improve patient care delivery, enhance the security of the patient medical records and computer workstations. Design clean storage space, expand exam room facilities, and improve wheelchair space to accommodate patients safely. Secondary focus shall be the complete design of Optometry Section space on Wards 8A & 8C, Advanced Low Vision Clinic and VIST (Visual Impairment Svc Team Blind Rehab) on Ward 8B using the VA Space Criteria as a guide including upgrading architectural, plumbing, HVAC, electrical systems to improve patient flow, environment and infrastructure. Tertiary focus shall meet the needs of the Ophthalmology, Optometry, Advanced Low Vision Clinic and VIST (Visual Impairment Svc Team Blind Rehab) as delineated during project meetings and site surveys using the VA Space Criteria as a guide. Quaternary focus shall be to correct building space deficiencies to the extent that the budget permits The selected a/e will be required to prepare 100% working drawings and contract documents using VA Master Specification (www.va.gov/facmgt/standard/spec_idx.asp) to complete the work specified above. The estimated construction range is $1,000,000 - $5,000,000. Design reviews will be due at 30%, 60% and 90%. Final submission will be due in 135 calendar days of contract award. Selection criteria shall be in accordance with Federal Acquisition Regulation (FAR) Part 36.602-1 and VA Acquisition Regulation (VAAR) Part 836.602-1. Request for Proposal # VA-241-08-RP-0315 has been assigned and should be referenced on all correspondence regarding this announcement. NAICS Code 541310, Architect Services, is applicable to this acquisition; the size standard is $4.5 million. This acquisition is set aside for small business. The size standard for a firm to be considered as a Small Business is no greater $4.5 million. Service Disabled Veteran Owned and Veteran Owned Small Business firms under the applicable NAICS Code above are encouraged to submit their qualifications. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract.
