- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation number: FA5613-08-T-0024 issued as a request for quotation (RFQ). (iii) Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-25. (iv) Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 611699. (v) The 700 Contracting Squadron, LGCA has a requirement to purchase the following service: Description of requirements for the items to be acquired: CLIN 0001 LIFE SAFETY CODE & ESSENTIALS (NFPA 101) FOR HEALTH CARE OCCUPANCIES TRAINING Quantity 1 each. (vi) Description of requirements for the service to be acquired: 1. Non-Personal Services: The contractor shall provide all parts, tools, labor, materials, equipment, transportation, and personnel necessary to provide Life Safety Code and Essentials Training Class for Landstuhl Regional Medical Center, Facility Management Division (FMD). 2. General: As facility managers, the LRMC FMD Staff are the professional link between the facility occupants, the systems engineers, and various contractors. With the increasing demand for economy and energy conservation, the challenge for property and facility managers today is to create and maintain an efficient and safe building environment. 3. a. Above mentioned training class should provide the students with the information needed to make management decisions about various building systems and sites - decisions that will increase occupant safety and comfort, while facilitating building efficiency. b. A total of four (4) days instruction between 25 and 28 August 08 at Landstuhl Medical Center / Germany, are required with approx. 15 to 25 students per class. 4. Major topics to be covered in this course: General Requirements. Means of Egress (Basics) Means of Egress (Fundamentals) Means of Egress (Awareness) Features of Fire Protection Special Provisions and Operating Features Building Rehabilitation Business Occupancies Ambulatory Health Care Occupancies Fire Safety Evaluation System (NFPA 101A) 5. At the end of the course the students should have learned following key concepts and skills: Understanding of the requirements for the Life Safety Code The ability to recognize key life safety risks. The knowledge to maintain secure and safe buildings. 6. The contractor shall: Provide all personnel and supervision necessary to develop and implement Life Safety Code and essentials (NFPA 101) for health care facilities. The Contractor shall present the training in English language. The contractor shall provide each student with a training hand-out. The contractor shall issue a certificate of achievement/training upon successful completion of training-test and submit it within 45 days to student. The contractor shall hand-out a training critique form for each student to evaluate the teacher's performance and course effectiveness (see attached sample). Upon completion, this form shall be forwarded to the Point of Contact (POC) within 30 days after training. 7. The Government will provide appropriate classroom space and equipment, such as a flip chart, CPU, overhead projector and a test proctor if needed to be coordinated with the POC after award of contract. 8. Performance Period: Between 25 and 28 August 08; exact time will be coordinated with the POC and the contractor. Eight hours of instruction per day from 07.30 through 16.30 hrs with 10 minutes break every class hour and a one (1) hour lunch break determined by instructor. Testing if needed to be conducted last 2 hours of class on fourth (4) training day. 9. (ix) The following Clauses and Provisions apply to this requirement: DFARS 252.204-7004 Alt. A. Award will be made to the technically acceptable low responsible offer. (xi) Include a completed copy of the provision at 52.212-3, Alt I, Offeror Representations and Certifications -- Commercial Items, with your offer or complete paragraph (j) of 52.212-3 Alt 1, if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov.
