- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation VA-256-08-RQ-0310 issued as a 100% Small Business Set-Aside Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-25. No telephone offer will be accepted. Only written offers received directly from the requester are acceptable. The North American Industry Classification System (NAICS) Code 423450. This size standard is 100 Employees. OFFERORS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. OFFEROR TO SPECIFY WHETHER QUOTING ON BRAND NAME ONLY OR BRAND NAME OR EQUAL IF QUOTING ON BRAND NAME OR EQUAL, DESCRIPTIVE LITERATURE SHALL BE PROVIDE WITH YOUR OFFER OR SPECIFY. Items 0001 - Purchase of Braeside Model QMR-4000 Deluxe Floor Standing Respiratory Display with Acrylic Components or Equal. Display to be (a) Unassembled; (b) Dimensions of Bottom Base - 23"W x 18.5"D; (c) Overall weight of full assembled unit - 53 lbs; (d) Solid Oak and Oak Veneer components; (e) Oak components with one of three stock finishes; (f) Double-facing, top-loading graphic header for featuring changeable 22"W x 28"H graphic - Opposing graphic header includes two panels for featuring 22"W x 28"H poster insert (i) One clear 0.80" plastic cover panel 22"Wx28"H (ii) One white 0.060" plastic backer panel 22"Wx28"H; (g) Three separate black, PVC plastic dispensers mounted on lower convex wood panel of one side of fixture. Incorporated herein by reference the following FAR Clauses and Provisions apply to this acquisition: 52.211-6, Name Brand or Equal (AUG 1999); 52.212-1, Instructions to Offerors - Commercial Items (APR 2008) - offerors must comply with all instructions contained in this provision; 52.212-2, Evaluation - Commercial Item (JAN 1999) - The Government will award a contract resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technically acceptable (name brand or equal) (2) Price and (3) Delivery. Technically acceptable is more important than price and delivery. To be technically acceptable the contractor must specify whether quoting on brand name or "or equal" items. If quoting on "or equal" descriptive literature must be provided with your quote. Delivery will be considered as of equal importance with price. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material which are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside the continental United States.
