- This is a combined synopsis/solicitation for commercial itemsprepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announce-ment constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued separate from this announcement. Solicitation DE-RQ65-08WG79093 is issued as a Request for Quotation. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-25. A Blanket Purchase Agreement will be awarded using Simplified Acquisition Procedures. This solicitation is Full and Open. The North American Industrial Code (NAICS) is 621111 and the business size standard is $8.5 Million. COMMERCIAL ITEM SPECIFICATION OCCUPATIONAL MEDICAL EXAMINATION SERVICES The U.S. Department of Energy, Western Area Power Administration, Desert Southwest Region requires the following: SEE ATTACHED BID SCHEDULE AND PERFORMANCE WORK STATEMENT Specific address/directions will be given upon award. Period of performance shall be a base year with two (2) option years [The base period of performance shall be 12 consecutive months from the effective date shown on Award document. Each option year (1st option year through 2nd option year, if exercised by the Government) will commence upon the effective date of option exercise and continue for 12 consecutive calendar months]. The following FAR clauses/provisions are applicable to this acquisition and all are attached. FAR Clause/Provisions: 52.212-1, Instructions to Offerors -- Commercial, 52.212-2, Evaluation -- Commercial Items, 52.212-4, Contract Terms and Conditions -- Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items. Offerors should include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items, found at http://farsite.hill.af.mil/vffara.htm or, complete a copy in the ORCA database http://orca.bpn.gov. If a change occurs in this requirement, only those offerors that respond to this announcement within the required time frame will be provided any changes/amendments and considered for future discussions and/or award.
