Description
This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures (Dollar Threshold of $100,000), a fixed-price purchase order at a fixed labor rate for a medical expert in issues of gerontology and medical treatment of older persons who also has expertise and experience with medical direction in nursing homes, nursing home policies and procedures, and the long term care regulations and survey process. A purchase order award will be made within (15) days after the response due date of this notice. It is anticipated that the period of performance will be September 29, 2008 – September 28, 2009. The estimated level of effort for this procurement is 500 hours. The North American Industry Classification System Code (NAICS) is 541990. The Statement of Work and Deliverables are attached. This Request for Quotes (RFQ) is being set aside for small business concerns. Responses must be sent electronically in MS Word format to Lucille.Lee@cms.hhs.gov and in hard copy no later than 11:00am on July 7, 2008 to Lucille Lee or Louis Anderson, Office of Acquisitions and Grants Management, Centers for Medicare and Medicaid Services, 7500 Security Blvd., Baltimore, MD 21244. Responses shall be no more than five pages and shall include the following: 1) Contractor’s plan to perform the Statement of Work; 2) Past performance information - relevant experience to perform the required services; and 3) fully loaded labor rate and anticipated hours. All responses will be evaluated on a best value basis using the following Evaluation Criteria: 1) Experience providing medical director services in nursing homes and working with homes to develop/revise their medical and clinical policies and procedures; 2) Knowledge concerning the Federal nursing home regulations; interpretive guidelines and survey processes; 3) Experience in the development and presentation of dvd-based and paper-based training course materials/clinical topics; 4) Experience providing editorial markup/comment on draft documents; 5) Past Performance; and 6) Price. The government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price; the best value (considering price and other factors). Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict or the appearance of a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated.
