Description
COMBINED SYNOPSIS/SOLICITATION (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number FA7014-08-T-A007. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. (IV) This procurement is 100% small business set aside. The associated NAICS code is 517110. The small business size standard is 1,500 employees. (V) This combined solicitation/synopsis is for purchase of the following commercial item(s)/services: 0001. Maintenance & Repair for Video Teleconference systems located at Malcolm Grow Medical Center on Andrews Air Force Base. MD. The purpose for the systems is to provide teleconferencing systems that allow for meetings and organizational units to conduct professional briefings. The period of performance will be for twelve months following date of award plus four one-year options. OPTIONS: 1001. Year 1 Maintenance & repair for video teleconference systems 2001. Year 2 Maintenance & repair for video teleconference systems 3001. Year 3 Maintenance & repair for video teleconference systems 4001. Year 4 Maintenance & repair for video teleconference systems (VI) Description of requirements is as follows: SCOPE OF NON PERSONAL SERVICE The contractor will provide maintenance and repair (including software upgrades) to the following video teleconference systems: Make Model Serial Number Tandberg 6000 MXP 115880V01D Tandberg 3000 MXP 114144V01D Tandberg 1500 MXP 113920V01D Tandberg 1000 112100V01D The contractor will interact with the 79 MDSS/SGSI Chief of Communications, Network Liaison, and/or his designated assistant for all work tasks assigned by the Quality Assurance Personnel. BACKGROUND The Malcolm Grow Medical Center (79 MDG) located at Andrews Air Force Base, MD provides a full and complete range of inpatient and outpatient medical services to the operating forces, beneficiaries, retired personnel and other appropriate personnel within the Washington, DC and eastern Maryland operations areas. The Medical Information Management Flight (79 MDSS/SGSI) provides a full range of information management/information technology (IM/IT) products and services to 79 MDG and satellite clinics. These services include, but are not limited to, automated information systems (AIS) and visual information systems (VIS), implementation, operations, training, maintenance, repair, and VTC network design. SERVICES DESCRIPTION: The contractor shall be directly responsible for ensuring the accuracy, timeliness, and completion of all tasks under this effort. The contractor shall provide travel, tools, supplies, parts, labor and other expenses necessary to maintain and repair the four video teleconferencing systems located at Malcolm Grow Medical Center on Andrews Air Force Base, MD. The purpose for the systems is to provide video teleconferencing systems that allow for meetings and organizational units to conduct professional briefings. ADPE MAINTENANCE SERVICES Services of equipment shall be rendered according to the equipment manufacturer's recommendations. The equipment covered under this PWS is currently in good operating condition. The contractor shall provide telephone troubleshooting support, on-site, on-call maintenance, preventative maintenance inspections and corrective service in support of the Video Teleconferencing Systems identified on the schedule. Repairs made shall be warranted for a minimum of 30 days by the contractor. All equipment placed under this agreement is in good operating condition. The contractor shall provide all necessary parts and labor as required to perform on-site maintenance and telephone support during normal business hours 8:00 AM to 5:00 PM five days a week. The contractor shall report to the job site within twenty-four hours after notification of the problem/outage. Maintenance shall include: Perform preventative maintenance inspections on the installed systems and the equipment end-items comprised therein. Preventative maintenance inspections shall include: Verify the operating conditions of all system elements; determine and perform any necessary adjustments, realignments, cleaning or repair actions required. Verify the operating conditions of all system integration elements, including, but not limited to, cabling, signal distribution and system control functions. Test all functions of system integration control and correct as appropriate. Check mechanical systems Check specific operating items, clean and align as necessary. Perform complete mechanical and electronic focus, CRT convergence, and gray scale to all CRT projectors, inclusive of shading for ILA products. Projectors will be aligned and adjusted for all sources available at the time of the Preventive Maintenance (when applicable). Test audio reinforcement systems for hum, buzz or other extraneous noise as per installation standards. System equalization and output levels shall be checked in accordance with installation standards. Consult with Quality Assurance Personnel on system status. Identify any specific corrective repair actions. Consult with Quality Assurance Personnel on system use, operation technique, training, or enhancement. Perform color balance and shading on multi-projector system (when applicable). Perform system performance demonstration, as requested. The software, entitled Presenter, is presently installed on the VTC systems. The contractor shall provide upgrades of the software to ensure that the video teleconference systems work properly. Contractor shall provide, as requested by the Quality Assurance Personnel, on-site instruction on the proper use of system equipment. (VII)Place of performance is, Malcolm Grow Medical Center, Andrews Air Force Base, MD (VIII) The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (Nov 2007), with its quote. (IX) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items (Feb 2007), applies to this acquisition. (X) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (April 2008) applies to this acquisition.
