Description
The Department of Veteran Affairs at Veterans Affairs Northern California Health Care System, is seeking sources under NAICS Code 621111(size standard of $9M) that are classified as either: 1) 100% Service Disabled Veteran Owned, or 2) Small Business that can provide medical evaluation examinations for our Compensation and Pension Examination Program. Moreover we are seeking information regarding availability and capacity to cover multiple locations. Large Businesses are welcome to respond for informational purposes in the event no Small Businesses have the capacity to provide this service at multiple locations. Contract services will be provided at various contractor owned facilities within Northern California in proximity (within 20 miles) of the following locations: Concord, Oakland, Chico, Redding, Martinez, and Sacramento. Multiple awards may be made to cover all areas. Contractors responding to this Sources Sought Notice need to provide the following information to diane.ferguson@va.gov no later than June 30, 2008. For socioeconomic status: Company Name, Address, Phone Number, Point of Contact Three Reference Points of Contact for similar work Previous Hospital/Medical Clinic Project Experience within last three years For contract coverage: List of areas that contractor can cover under the contract Location and type of facility Indicate whether facility is contractor owned or subcontracted The Contractor shall provide the facilities and physicians to perform examinations and evaluations for qualified referred veterans who have submitted a claim for a service connected disability rating. The evaluations will range from routine general medical examinations to highly complex medical examinations, requiring a thorough evaluation and review of extensive history, diagnostic acumen, in subspecialties such as Orthopedics, Mental Health, Neurology, Audiology and in general medicine. The contracted physicians would need to be available Monday thru Friday, 8:00 am to 4:00 pm to accept appointments.
