SOLICITATION NUMBER: W67K2S-8161-0500 QUOTE DUE DATE: 7 July 2008 POC: Linda Duvall, Contracting Officer, (801)523-4091 DESCRIPTION: This a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are hereby requested. A written solicitation will not be issued. This requirement will be 100% Small Business Set-Aside. The North American Industry Classification System (NAICS) Code will be 611620, size standard is $6.5M Employees. REQUIREMENT: 1. Yoga Instructor every Monday and Wednesday 11:00 a.m 12:00 p.m. MST (except on Federal Holidays) at the Utah National Guard Headquarters, 12953 South Minuteman Drive, Draper, Utah. 2. Instructor must be certified in the Baptiste Method of Yoga. The Baptiste Method of Yoga was founded in the 1940's by Walt Baptiste and has constantly evolved to stay current with contemporary needs. The physical aspects of Baptiste Power Vinyasa Yoga style are inspired by the hatha yoga teachings of Krishnamacharya and his students Iyengar and Desikachar. Baptiste Power Vinyasa Yoga emphasizes training the mind, establishing self-esteem, opening up to grace, the integration of breath and movement, focus on technical and biomechanical knowledge, adapting each asana to unique individual needs, and building effective and clear themes that deepen the student's experience and understanding of a transformational personal practice. The Baptiste Power Vinyasa Yoga practice provides a metaphoric experience to discover more about personal power, self-esteem, personal boundaries, safety, self-acceptance, inner surrender and responsibility. Yoga and meditation practice are used experientially and metaphorically to encourage participants to reclaim their full potential, discover creativity, find passion, and experience confidence and caring about the body in order to live in higher alignment and to share their experience with others. 3. Potential instructors must provide how long their training was, how much practical experience they have with Yoga and must be certified in the Baptiste Method of Yoga. 4. If the instructor is not able to teach a session it will be their responsibility to find a replacement or schedule a make-up session. 5. Instructor must accommodate both beginner and advanced students. EVALUATION FACTORS: The contract type for this procurement will be Firm-Fixed price and award will be made on the basis of best value to the Government, price and other factors considered. Selection will be based on the following evaluation factors in no order of importance: (1) past performance, experience within the past year teaching Yoga; (2) certifications; and (3) total cost and/or price. ADDITIONAL INFORMATION: In accordance with FAR 52.212-1(k), prospective awardee shall be registered and active in the CCR database prior to award. If the offeror does not become registered in the CCR database by the due date/time of quote, the Contracting Officer will proceed to award to the next otherwise successful registered offeror. The base Period of Performance will be from 1 August 2008 31 July 2009. Each option year will be exercised if the contractor is performing to the satisfaction of the Medical Readiness Office and based on the availability of funds. Offerors must submit a quote for the base year and each option period. Quotes are due NLT 1:00 p.m. MST on 7 July 2008.
