Description
(i) This is combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The information we request below is the minimum information required for us to make an award. If the applicable information is not provided entirely, your quotation will not be considered for award. (ii) This solicitation is being issued as a Request for Quote (RFQ) No. FA5205-08-Q-KN07. Please submit a written quotation NLT 4 Jul 2008, 07:00 (Japan Standard Time) by Email. POC: Mr. Kazuhiro Nakamura, Email: kazuhiro.nakamura@misawa.af.mil, Tel: 011-81-176-53-6482. (iii) This solicitation document and provisions/clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26 (as of 12 Jun 08). (iv) The NAICS code 541620 applies to this solicitation. (v) Quotation is requested for asbestos and lead-based paint (LBP) training. 1) 4 day asbestos initial worker in Japanese up to 10 trainees; 2) 4 day asbestos initial worker in English up to 7 trainees; 3) 5 day asbestos initial supervisor in English up to 5 trainees; 4) 1 day asbestos refresher worker in Japanese up to 18 trainees; 5) 1 day asbestos refresher worker in English up to 4 trainees; 6) 1 day asbestos refresher supervisor in English up to 13 trainees; 7) a half day asbestos refresher inspector in English up to 5 trainees; 8) a half day asbestos refresher planner in English up to 3 trainees; 9) 1 day LBP refresher worker in Japanese up to 9 trainees; 10) 1 day LBP refresher supervisor in English up to 10 trainees; 11) 1 day LBP refresher inspector in English up to 4 trainees; and 12) 1 day LBP refresher risk assessor in English up to 4 trainees. (vi) Provide asbestos and lead-based paint trainings in English and Japanese at Misawa Air Base, Japan. The contractor shall issue certificates of each provided initial training and renew certificates of refresher trainings accredited by the state of Pennsylvania IAW U.S. Environmental Protection Agency (EPA) and the Occupational Safety and Health Administration (OSHA). The contractor shall have an EPA accredited training certificate or a state training certificate if the state has an EPA accredited program.(vii) Complete trainings within 21 calendar days in July and August 08 excluding Saturdays, Sundays and U.S. legal holidays IAW the Performance-based Work Statement (PWS). The contractor will be notified of the actual training dates and numbers of trainees for each training at least 21 calendar days before the first training date. (viii) The provision at FAR 52.212-1, Instructions to Offerors' Commercial Items, applies to this acquisition. (ix) FAR 52.212-2, Evaluation - Commercial Items (Jan 1999), (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Technical Capability: (1) Certificate of EPA or a state approved training provider; (2) Certificate of EPA or a state approved training instructor; and (3) Certificate of EPA or a state approved training course. (ii) Price: Offerors shall submit copies or supporting documentation of each certificate to the Government at the time of offer submission. Technical proposal will be evaluated to determine technical capability on a pass/fail basis, and the technically unacceptable offer will not be considered for award. Price of the technically acceptable offers will be evaluated. (b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (x) The provision at FAR 52.212-3, Offeror Represenations and Certifications-Commercial Items, applies to this acquisition. (xi) FAR 52.212-4, Contract Terms and Conditions- Commercial Items applies to this solicitation. (xii) FAR 52.212-5 (deviation), Contract Terms and Conditions Required to Implement Statues or Executive Orders- Commercial Items, applies to this solicitation. Specifically, the following clauses cited are applicable to this solicitation, FAR 52.222-19, Child Labor - Corporation with Authorities and Remedies, FAR 52.225-13, Restrictions on Certain Foreign Purchase, FAR 52.232-33, Payment by Electronic Funds Transfer -Central Contractor Registration. (Xiii) The above Provisions and Clauses may be obtained via the Internet at http://farsite.hill.af.mil. (xiv) Please provide the following information along with your quote; TIN, CAGE Code, DUNS No, quoter's name and title, e-mail address, company name, ordering address, Tel No. and Fax No. Date of Central Contractor Registration (CCR) Registration or Update, whichever is later.
