- This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities. The Oklahoma City Department of Veterans Affairs is seeking potential qualified contractors to provide a Capitated Community Based Outpatient clinic (CBOC) in or within a 30 mile radius of Ardmore, Oklahoma to service veteran patients. The contractor shall furnish health care providers, medical facilities, equipment and supplies, emergency-fill pharmaceutical prescriptions and administrative functions to fulfill the level of care and range of services necessary to meet the primary medical and mental health needs of enrolled patients The contractor shall be responsible for the administration and management of all aspects of this contract and the health plan covered there under, including responsibility for all employees, agents, subcontractors and any other entity acting for or on behalf of the contractor. Responses to this sources sought must include General Information regarding your company such as Name, Address, Contact Person, Phone Number, E-mail address, Type of business such as 8(a), small business, Hub Zone, Small Disadvantaged Business, Service Disabled Veteran owned or Veteran owned. In addition please indicate whether your company has experience with the Department of Veterans Affairs Capitated CBOC Program, and if it is your intention to be a joint venture with another company. Please indicate how long your company has been established, including square footage, number of employees, and description of facility.
