- Addendum 5 is changed to read "Attachment 1" Addendum 4.0, para. 5 should read "Wage determination No. 2005-2009 (Rev 6) dated 9 Oct 2007 is only applicable to proposed on-site Trainer Operators/Maintenance personnel under occupational code 30000 described in SOW para 1.2.4 and the on-site supply technician under occupational code 01000 described in SOW para. 1.2.5. See Attachment 1." Addendum 2, page 10, 1.2.1 Key Personnel is changed to read: For the purposes of this contract, the Site Manager and all onsite personnel shall be considered key personnel. Subsequent changes to key personnel shall require the contractor to submit a resume to the Contracting Officer with a copy to the Contracting Officer Technical Representative (COTR) for review and a determination of acceptability. All changes are posted to website http://www.uscg.mil/hq/arsc/ald.asp
