Amendment Two (2) HHSI161200800059C The following technical changes are hereby made to the specifications section: PART I LOCATION ESTABLISHMENT: DELETED 1. In the Performance Specification, the first paragraph of Section 2.02.G.2.A on page PS-19 should read as follows: The comfort air conditioning shall be designed and installed in accordance with the IMC, ASHRAE Standards, and any local or state supplemental codes or regulations having jurisdiction. The system shall include heat pump(s) or combination air condition/heating units as required for the building and be fully automatic. HVAC units may be heat pumps or electric cooling with electric heating. HVAC units shall be sized based upon the following design conditions for the selected site. 2. In the Performance Specification, Section 2.02.I.B on page PS-23 should read as follows: A. Contractor shall install 2" by 4" telephone/IT boxes at locations indicated on the plan set. B. Contractor shall install 3/4" conduit connected to the telephone/IT boxes running above the ceiling with pull strings inside the conduit. 3.REMOVE COMPLETELY: Scope of work for VOIP Telephone/IT specifications from the spec package. 4. In the Performance Specification, please delete Section 2.02.I.A.L on page PS-22, as that paragraph conflicts with the telephone/IT requirements above. 5. During the Pre-Bid, we were asked about fire flow. We cannot obtain flow data, as there is not a fire hydrant nearby. Please include the following: a. Contractor shall design the fire sprinkler system using the following available factors. The water storage tank has a bottom elevation of 4695 ft, a diameter of 20 ft, and an overflow height of 44.5 ft. The clinic has an average ground elevation of approximately 4676 ft. Flow data is unavailable. 6. During the Pre-Bid, we were asked about the overhead exam light in the Trauma Room. The exam light will be removed from the clinic with the demolition. No new overhead exam light is needed due to the use of hand-held exam lights. Amendment Two (2) HHSI161200800059C The following changes are hereby made to the solicitation section: Replace Section B completely and replace with new section B as follows: PART I THE SCHEDULE SECTION B SERVICES AND PRICE/COST Evaluation of proposals will be based on the following three requirements EVALUATION FACTOR ONE (1) 30 % of total award value The qualifications submitted by each firm will be reviewed and evaluated to identify the most highly qualified firm. When responding to this announcement, firms should fully address their capability and qualifications with regard to each of the following evaluation factors. Firms should submit their complete qualifications as they may be evaluated solely on their written responses to this announcement. (1) PAST PERFORMANCE CONTRACTS OF SIMILAR SIZE, SCOPE, AND COMPLEXITY WITH GOVERNMENT AND PRIVATE INDUSTRY. Construction experience on similar projects of similar size and scope. Elaborate on projects that have been completed in the last 3 years. Provide reference that may be contacted. Past performance on contracts with Government entities including Native American organizations, tribal governments, federal agencies, i.e., Indian Health Service and Bureau of Indian Affairs and Private Industry. (2) PROFESSIONAL QUALIFICATIONS AND EXPERIENCE OF THE PROPOSED STAFF FOR THIS PROJECT: QUALITY CONTROL/QUALITY ASSURANCE/ SAFETY EXPERIENCE DURING CONSTRUCTION PHASE: Professional qualifications of project managers, superintendents, estimator, quality control, and construction inspectors including professional registration, education, training, technical competence and skills as they relate specifically to medical projects. Description of specific safety and quality control initiatives and plans for this project. Provide information on your firms QA/QC procedures to ensure plans and specification are complete and accurate, including coordination of drawings between disciplines. Show how firm ensures applicable codes and standards are met in the design. (3) MANAGEMENT PLAN: Proposed project approach, construction method to accomplish contract requirements including teaming planning between contractor/sub-contractors, the designer of record and Government representatives. Cost management, critical path scheduling, value engineering and fast track experience must also be presented for consideration. Individuals submitted for evaluation may not be switched and must be used in their capacity on the project. EVALUATION FACTOR TWO: 40% of total award value. It is the intent of Engineering Services-Dallas to obtain a new health station which shall be a modular pre-engineered, pre-fabricated/pre-manufactured building transported to Ft. McDermitt, NV, and installed on a permanent foundation with a crawl space below. The modular medical clinic needs to be designed and constructed, within the parameters of the solicitation plans and specifications which shall be used as minimum requirements. The contractor will be required to design a complete building package providing the most amenities possible within the budgeted funds available conforming to the drawings and specification. The estimated project size is $450,000.00 dollars for both design and construction. Additional funding is not available. Our intent is to build a building obtaining the most we can with the money available. The specification provides the minimum technical engineering requirements and material performance standards for this health station. All design concepts will be considered that meet the specification and design documents. All construction plans shall be stamped by a Professional Engineer. Design shall include, site planning and drainage engineering, roads, walks, utilities and other infrastructure civil engineering, landscape design, architectural building design, structural engineering for buildings and other structures, mechanical engineering for building plumbing and HVAC systems, electrical engineering for building systems and site lighting, interior design, and any and all other architectural and engineering tasks as required for a complete project. PROPOSAL SUBMISSION REQUIREMENTS AND EVALUATION CRITERIA 1.General. The technical proposal will describe the capability of the offeror to perform any resultant contract. Provide the Contracting Officer with information sufficiently detailed to clearly indicate the materials, equipment, methods, functions, and schedules proposed. The proposal should be prepared simply and economically, providing straight-forward,concise delineation of capabilities to perform satisfactorily the contract being sought. 2.Format of Technical Proposals. No dollar amounts for the proposed work are to be included in the technical proposal. The technical evaluation criteria listed in this section should be addressed in order. The proposals are evaluated in direct correspondence to the technical evaluation criteria which are included in this section. If supplemental information relative to the criteria is included in another part of the technical proposal, its location must be identified. If the offeror fails to provide information relating to the criteria or locates the information in another part of the proposal without providing any cross references, the offeror runs the risk of having their proposal receive a lower evaluation by the Government evaluators who were not able to locate the appropriate information. A.The technical proposal shall include: 1.Title Page, including the title of the solicitation, solicitation number, offeror name, owner name, project number, and date of the submittal. 2.Table of Contents, including a list of tables. If more than one binder is used, the complete table of contents shall be included in each. Any materials submitted but not required by this solicitation, (such as company brochures), shall be relegated to appendices and may not be considered in the evaluations. 3.Narrative on design approach, including discussion on how the cultural sensitivity of the design has been addressed. 4.Compliance Statement: The offeror is required to certify that all item submitted in the technical proposal comply with the RFP requirements and any differences, deviations or exceptions must be stated and explained. Offerors are required to complete the statement and submit it with their technical proposal. Even if there are no differences, deviations or exceptions, the offeror must submit the Compliance Statement and state that none exist. B.Design Freedom. Requirements stated in this RFP are minimums. Innovative, creative, or cost-saving proposals that meet or exceed these requirements are encouraged and will receive quality points accordingly. Existing plans or modifications thereof that meet the design and construction criteria specified herein, which an offeror has previously constructed and priced, may be submitted. They may include designs incorporating factory fabricated components or modules. Deviations from space and adjacency requirements are discouraged unless the changes result in improvement to the facilities. TECHNICAL EVALUATION CRITERIA A.OVERVIEW OF TECHNICAL EVALUATION. Technical Quality Evaluation. Technical quality evaluation consists of an in-depth technical quality review of the proposals submitted. Each proposal will be awarded a total point score based on the grading procedure in this RFP. The technical quality evaluation is conducted by the evaluation team. The team identifies all issues that require further clarification. These issues may be resolved through discussions between the Government and the offerors. B.EVALUATION TEAM COMPOSITION AND PROCEDURES. The evaluation team is composed of three voting members representing The Indian Health Service Area and Service Unit as well as Engineering Services. C.SCORING. Proposals will be technically evaluated and assigned point scores based upon the factors shown in specifications and drawings. Evaluation will be based strictly on the requirements stated in the STATEMENT OF WORK (SOW) and the PROPOSAL EVALUATION CRITERIA as shown in this RFP. Decisions and recommendations of the evaluation team will be by consensus of the three voting members. Point Distribution. Although any number of points between zero and the maximum can be awarded to a given element, the distribution of points should reflect the following parameters for consistency: 1.25% Element barely meets minimum stated RFP criteria or has minor correctable deficiencies, but offers no advantage to the Government over the basic RFP requirements. 2.50% Element meets the RFP criteria, and contributes to a coordinated design approach which will produce an acceptable level of quality. 3.75% Element meets the RFP criteria, and has salient features that offer advantage to the Government in terms of design quality, potential energy cost savings, potential maintenance cost savings, or ease of Government administration. 4.100% Element meets the RFP criteria, and has salient features that offer considerable advantage to the Government in terms of design quality, potential energy cost savings, potential maintenance cost savings, or ease of Government administration. EVALUATION FACTOR THREE: 30% percent of award value Pricing will be established as designing within fee limitations and the final construction lump sum price will be negotiated. Contractors will be allowed a 3% contingency in their final price for unexpected costs or cost overruns. Approval of use of the contingency funds will be thru the Contracting Officer. At the conclusion of the project unused contingency monies will be divided 50/50 between the Government and the contractor. The Contractor's proposed price shall be all inclusive to provide for all labor, material, supplies, transportation, supervision and all other necessary items for a complete and usable project, performing all work in a workman like manner in complete compliance with all plans, drawings, and specifications approved by the Government. Work is all inclusive for complete and useable systems and price proposals must contain all associated costs. I ITEM ONE: OFFERORS ARE REQUIRED TO PROVIDE A TOTAL LINE ITEM BREAK DOWN OF ALL COSTS INCLUDING OVERHEAD AND PROFIT SHOWING HOW THEY ARRIVED AT THE TOTAL LUMP SUM PROPOSED PRICE. THE GOVERNMENT WILL USE THESE FIGURES TO EVALUATE CONTRACTORS COSTS AND TO ASCERTAIN THAT ALL FACTORS HAVE BEEN TAKEN INTO CONSIDERATION. TOTAL LUMP SUM PROPOSAL: $______________________________ (Attach total Breakdown) Failure on the part of the offeror to submit the require cost breakdowns could result in the offer being rejected as non-responsive. The Government reserves the right to reject any or all proposals prior to award; to negotiate with any or all offerors; to award the contract to other than the lowest total price; and to award to the offerors submitting the proposal determined by the Government to be the most advantageous to the Government price and other factor considered. Award of a firm fixed price contract will be made. THE PROPOSAL DUE DATE OF JULY 17, 2008 AT 2:00PM CENTRAL TIME REMAINS UNCHANGED.
