- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W81NTE81288501 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. This acquisition is set-aside 100% for small business under NAICS code 334517 and the small business size standard is 500 employees. The requirements in this solicitation are: CLIN 0001, 2 ea HeartSuite Hemodynamics Record Station Upgrade HH-10274, CLIN 0002 Dell 2900 LT02 Backup Kit Includes 3 LT02 Media HH-10346, CLIN 0003, 1 each Dell 2900 Rack Conversion Kit HH-10345, CLIN 0004 2 ea Serial wireless bar code scanner PH-10138, CLIN 0005 1 ea Server 2003 Operating System Upgrade HH-10278, CLIN 0006 1 ea Standard Server Hardware Refresh for HeartSuite Hemo HH-10344, CLIN 0007 PhysioLog Database Migration to New Server PH-10169, CLIN 0008 2 EA Hemo PC Hardware Refresh for HeartSuite Hemo Record Station HH-10332, CLIN 0009 Client PC Hardware Refresh for HeartSuite Hemo Record Station HH-10333, CLIN 0010 Keyboard to Serial Bar Code Upgrade HH-10277 Keyboard to Serial Bar Code Upgrade, CLIN 0011 Physiolog Installation, CLIN 0012 Physiolog Training, and CLIN 0013 Service Contract. Federal Acquisition Regulation (FAR) Clause 52.211-6 Brand Name or Equal applies to this acquisition. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB destination to Brooke Army Medical Center, 3851 Roger Brooke Drive, Fort Sam Houston, Texas 78234-6200. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Lowest Price, Technically Acceptable (compliance with the requirements of FAR Clause 52.211-6 if vendor offers an Equal Product. Vendors shall include a completed copy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation.
