Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W911RZ-08-T-0173 is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-25, effective 22 April 2008. This acquisition is a 100% set-aside for Service Disabled Veteran Owned Businesses. This request for Quote is due by 09 July 2008, 12:00 PM (Mountain Time). The RFQ will result in a firm fixed price contract. Quotations will be accepted through email or fax only. Quotes can be submitted to ladonna.collins@us.army.mil or faxed to 719-526-4490. The North American Industry Classification System (NAICS) code for this acquisition is 611519. The Directorate of Contracting located at Fort Carson, Colorado is soliciting quotes to procure the following services: EMERGENCY MEDICAL TECHNICIAN (EMT) BASIC COURSE CLIN 0001 30 personnel during the following time period: 14 July 11 August 2008 CLIN 0002 60 personnel during the following time period: 08 Sept 03 October 2008 The successful all inclusive offer will provide a four-week accelerated accredited college credit course that will include tuition, training facilities, course instructors, personal equipment, textbooks and all necessary content and required training materials, EMS student uniform, Emergency Medical Services (EMS) Student Insurance, EMS Student Photo ID, Clinical Ride Time, CPR card and Certificate of Completion. The four-week accelerated course will consist of five sessions per week lasting from 9:00am 5:00pm each session with a one hour allocated lunch break. Students/Soldiers will retain textbooks and training resources as listed. The Government will allow costs only for the actual number of students/soldiers that are physically present for the training. Offerors will provide copies of teaching certificates for all instructors and copies of national accredition. The students must be certified as Emergency Medical Technicians with a certificate that will be recognized by the U.S. Army. Proposal shall include 1). Total proposed all inclusive cost 2). Agreement to the Period of Performance 3). Copy of EMT Instructor certification 4). Copy of Accreditation Documentation. Evaluation factors are as follows: 1). Low cost to the Government 2). Specify accomplishment of requirements within the stated period of performance. 3). Provision of required instructor EMT certification for the required training. 4). Provision of proper accreditation. $_____________________ LUMP SUM BID Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to OfferorsCommercial Items. FAR 52.212-1 is supplemented per following: Contractor shall submit their quote on company letterhead, provide solicitation number, the time specified in the solicitation for receipt of offers, name, address and telephone number of the offeror, unit price, an over all total price to evaluate compliance with the requirements in the solicitation, any discount terms, cage code, DUNS number, acknowledgement of solicitation Amendments (if any). The offeror must be registered on the Central Contractors Registration database (CCR). Information concerning CCR requirements may be viewed via the internet at http://www.ccr.gov or by calling the CCR registration centers at 1-888-227-2423. Only contractors who are registered in the Central Contractor Registration (CCR) can be awarded a contract. Offerors must have electronic funds transfer (EFT) capability. Offerors proposals shall be valid for a minimum of 60 days to be acknowledged in the offerors proposals. 52.212-2 Evaluation of Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be the lowest priced offer.
