- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number: W9114F-08-T-0099 issued as a request for quotation (RFQ). (iii) Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. (iv) Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 423450. Only one award will be made for this requirement. (v) The Europe Regional Contracting Office has a requirement to purchase the following items: Description of requirements for the items to be acquired: CLIN 0001 Grafton Paste, 10cc, P/N: 5010, 20 EA; CLIN 0002 Grafton Paste, 5cc, P/N: 5005, 10 EA; CLIN 0003 Grafton Paste, 5cc, P/N: 5001, 15 EA. Product offered must be FDA approved. Manufacturer: Osteotech, Inc. Brand name or equal (vii) The place(s) of delivery and acceptance will be at the following addresses: U.S. Hospital Landstuhl, BLDG 3740, 66849 Landstuhl, Germany. All offerors must quote FOB Destination, Landstuhl Regional Medical Center, Landstuhl, Germany. Delivery date: 2 weeks ARO. (viii) The following Clauses and Provisions apply to this requirement: FAR 52.204-7.
