Description
The USDA, Forest Service, Olympic National Forest, intends to acquire a prefabricated bridge superstructure, approximately 50 feet in length, and approach ramp, approximately 18 feet in length, for the Lake Cushman Temporary Bridge. Contractor shall design, fabricate and deliver the bridge superstructure and approach ramp. Lake Cushman Bridge is a reinforced concrete girder frame bridge 200 feet long with 5 spans of lengths 12 ft, 53 ft, 70 ft, 53 ft, and 12 ft. The end spans are cantilevered. The substructure consists of four reinforced concrete bent piers, all of which are pile supported. End bents have 6 steel H-piles in a single line with a reinforced concrete cap. Interior pier bents have two lines of 6 steel H-piles each with a concrete cap supporting a solid shaft pier. The deck is reinforced concrete with no additional wearing surface. The Lake Cushman Bridge was damaged during a winter storm in December, 2007. The prefabricated, portable bridge superstructure and approach ramps will be made to the attached specifications and drawings. The steel superstructure will be fabricated in an AISC certified plant. A solicitation will be issued under FAR Part 12 for a commercial supply item. This is a small business set-aside. NAICS code 332312: Fabricated Structural Metal Manufacturing; 500 employees. The Lake Cushman Bridge is located in Washington on the Olympic National Forest on the Hood Canal Ranger District in Mason County and further located in T. 23N., R. 5 W., Section 10. From Olympia, travel north on Hwy 101 to Hoodsport, then northeast on WA-119 North Lake Cushman Road to the intersection with Forest Road 24. Turn left at the ‘T’ intersection onto Forest Road 24 and travel east until the intersection with Forest Road 2451. The bridge is located at M.P. 0.2 on Forest Road 2451. The delivery site will be near the Lake Cushman Bridge. The delivery site is approximately 17 miles from Hoodsport, Washington.
