- THIS IS NOT A REQUEST FOR PROPOSAL and NO SOLICITATION PACKAGE OR BIDDER/PLAN HOLDER LIST WILL BE ISSUED. This solicitation is changed to full and open procurement and the mileage restriction is increased to 120 miles from the VA Fort Wayne, IN.VA Northern Indiana Health Care System (NIHCS) is seeking a qualified Architect- Engineering (A/E) firm with specialty and/or experience working with VA Health Care facilities to provide a complete A/E design for Project No. 610A4-08-124, Modifications to Surgery Utility Room for the VA NIHCS, 2121 Lake Avenue, Fort Wayne, IN 46805. A/E to design a construction project for the interior renovation of existing surgery utility/storage rooms to clean and soiled utility rooms and to provide construction period services. Design will include interior demolition, new finished construction, plumbing relocation, electrical service revision, and air handling system modification. Phasing will be critical as all work will take place in an active surgical suite. Plans and specifications shall include detailed requirements on the accomplishment of the work to minimize the negative impact on the surgical suite. A/E will generate a complete set of AutoCAD working drawings, specifications, and cost estimate. A/E to provide at least one set in reproducible hard copy and one set in electronic format that is compatible with systems. An initial site visit will be required by the A/E for verification of existing conditions and measurements. A/E Construction period services will include reviewing contractor's material submittals for accuracy and completeness and periodically conduct site visits to review the ongoing construction. Estimated construction cost is $90,000. Estimated cost for the design and construction period services is under $25,000. Projected design completion time is 75 days. A/E shall provide all architectural and engineering services as necessary to complete the following phases: design development, construction document, and construction period services. Design Development and Construction Document includes all work necessary to develop detailed construction documents (drawings, specifications, cost estimates, and phasing plans) and attendance at review meetings. A/E shall follow the A/E Submission Instructions to be provided in the contract. Construction period services in the form of clarifications; submittal review; resolution of design conflicts; additional design work due to changes, differing site conditions, or errors; periodic and/or scheduled inspections; and meeting attendance. The A/E shall be responsible for the professional quality and technical accuracy of all of the documents it prepares, and a design narrative/analysis, calculations and edited VA specifications in accordance with professional standard practices. All work shall comply with current VA Master Specifications and Standard Details. Design shall be prepared in the most recent version of AutoCAD available, as a minimum the drawings shall be compatible with AutoCAD version 2007. Specifications shall be provided in electronic format, and shall be prepared in Microsoft Word version 2003 as a minimum. Firm must be located within or about 120 miles of the VA NIHCS, Fort Wayne, IN 46805. Hard copy to be submitted to: VA NIHCS (90C), Marjean Litzenberg, The A/E selection criteria shall include: 1) proposed management plan; 2) professional qualifications of proposed team; 3) previous experience of proposed team; 4) project control - techniques used, personnel and quality control; 5)estimating effectiveness; 6) proposed design approach; 7) Location and facilities of working offices; 8) team design philosophy and method of implementing; 9) miscellaneous experience & technical competence; 10) awards for design excellence, and 11) insurance coverage and litigation history. The NAICS Code for this acquisition is 541310 - Architectural Services and the applicable Small Business Size Standard is $4.5 million dollars average annual gross revenues for the past three fiscal years), The awarded AE must be registered in the Central Contractor Registration (CCR) database (A/E firms can obtain CCR information on registration and annual confirmation requirements by calling 1-888-227-2423, or 269-961-5757, or via the Internet at http://www.cc r.gov.) and is required to submit Annual Representations and Certifications via the Online Representations and Certifications Application (ORCA) at https://orca.bpn.gov/.
