- This is a combined synopsis/solicitation by the U.S. Census Bureau in Jeffersonville, Indiana, for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as an indefinite quantity/indefinite delivery Request for Quote (RFQ). Reference RFQ No. 51-SO-BC-8-00036. (iii) This RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26 dated June 12, 2008. (iv) This action is set-aside for Small Businesses. The applicable NAICS code is 423830. The threshold is 100 employees. (v) This action is to acquire an indefinite quantity (minimum/maximum) of the following line items: Line item 0001.) Pallet Positioner and Scissors Lift Units, min. 6 EA. max. 18 EA. Line item 0002.) Conveyance Devices (Trays), min. 1,146 EA. max. 4,002 EA. Line item 0003.) Work Table, min. 12 EA. max. 72 EA. Line item 0004.) Anti-Fatigue Mats, min. 60 EA. max. 360 EA. Line item 0005.) Sit-Stand Stools, min. 60 EA. max. 360 EA. (vi) Line item 0001, Pallet Positioner and Scissor Lift, Southworth Palletpal Powered-Hydraulic, Model #PPH2-24, or equal, includes electric powered hydraulic lift (no shop air available), standard electrical - 115/1/60 power supply, foot controls for raising & lowering, standard pallet size 42” diameter, minimum (<45”), low friction manual rotation capability, lift adjustable at multiple heights with 30-36 inches lift range, portable fork pockets for safety and ease of relocating with a fork lift truck, load capacity-2,000 pounds minimum, optional-4,000 pounds. Line item 0002, Conveyance Devices (Trays) for accumulating forms and supplies as the tray moves along an assembly line from station to station as assembly personnel add materials and/or forms to the tray, creating a “kit” of required forms and supplies. Specifications: Inside measurements: 12” wide x 18” long x 2” deep (min.) to 6”(max); such as ULINE model S-132. It shall have tapered side and end walls to provide “nesting” capability, lightweight and durable, and grip handles located on the ends or sides, construction similar to USPS mail handling trays. Line item 0003, Work Tables, specifications: industrial work surface packing table with composite wood top, 48”long x 30”wide, adjustable height: bolt together legs at prescribed height. Example: ULINE Industrial Packing Table Model #H-1128, with composite wood top. Line item 0004, Anti-Fatigue Mats, Uline Model #H-331 or equal, to provide a comfortable surface inside a workstation to provide anti-fatigue relief for people. Specifications: sturdy, long life anti-fatigue matting, 24” width x 36” length, beveled edges to eliminate tripping hazards, and smooth top surface to accommodate both standing and worker use of a sit-stand stool whose base includes stabilizing glides. Line item 0005, Sit-Stand Stools, to provide ergonomically designed support device that allows the user to work in an upright, standing position while most of his or her weight rests on the padded seat, taking the load off the worker’s legs and feet. Also small enough to allow the worker to stand as desired while working. Base design allows for positioning up close to workstation point of use, is stable but not bulky, has anti-slip glides on the base for improved stability, has ergonomically formed seat, padded, with swivel feature (up to 20 degrees, left and right) with automatic return to center, and seat tilt up to 15 degrees. Seat height is adjustable from 23-1/2” to 33-1/2”. Refer to Lyon “Sit-Stand Stool” or equal. (vii) This is an indefinite-quantity contract for items and is in effect through 31-MAR-2008. The quantities of items in the schedule are estimates only. The Government will order at least the quantity of supplies designated in the schedule as “minimum”. Delivery of the balance will be on an as needed bases with the Government giving a two-week notice of delivery. Anticipated delivery is for all minimum requirements to be made no later than 03-OCT-2008. All units will be delivered, FOB destination, to Bureau of the Census, Receiving-Bldg. 60F, 1201 East Tenth Street, Jeffersonville, Indiana, 47132, or a location within a ten mile radius of this location, in Clark County, Indiana. (viii) FAR provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. (ix) FAR provision at 52.212-2, Evaluation-Commercial Items, is included with the following factors to be applied for evaluation; 1.) conformity with the specifications and performance abilities of the unit specified, 2.) ability to meet delivery requirements, 3.) warranty and service support/replacement programs and procedures, 4.) on site evaluation of sample units to be provided on a trial basis at no cost to the Census Bureau, including shipping and handling costs, if the need for sample units is determined to be necessary. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. (xi) FAR clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xii) FAR clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. (xiii) Offers must be registered in CCR (www.ccr.gov). (xiv) Offers are due NLT 2:00 PM, EST, Thursday, 14-AUG-2008.
