Description
Contractor shall furnish all labor, equipment, parts, travel, software, and all other associated costs necessary to inspect, clean, adjust and perform preventative maintenance and routine emergency service on Steris on 130 Cart washer ser #3602607003, Reliance Endoscope Processor ser #3611507008, and Reliance Endoscope Processor ser #3630606006 A minimum of two PM inspections to be performed per year. Preventative Maintenance and routine/emergency service for above named equipment Shall be provided for the duration of this contract. DESCRIPTION/SPECIFICATIONS/WORK STATEMENT Contractor shall provide all routine repair parts, supplies, software, travel, shipping and Emergency service, for an all-inclusive contract price in order to repair/adjust Steris 130 Cart washer, and Endoscope Processors (2) and perform two (2) preventative maintenance calls per contract period. This service shall be the same as that provided to Other users of the same equipment under the OEM Platinum agreement. Coverage hours are Monday thru Friday 8am to 5pm excluding federal holidays. Response time 1hr phone on site in 4 hours unless other arrangements made with COTR. Emergency repairs require a response time of 24 hrs or less, which can be accomplished by one of the following. 1.A telephone trouble shooting call with Biomed 2.Dispatch of parts to resolve problem (with agreement of COTR or representative) 3.On site visit. "Response time" is defined as the time inclusive from notification of the problem to arrival at the VA Medical Center, Cheyenne, Wyoming by the Contractor. Failure to comply with this "Response Time" may result in default actions being taken against the Contractor. The preventative maintenance visits must be accomplished within Sixteen (16) working hours of arrival on site. CONTACT POINT FOR SERVICE CALLS: (to be completed by the Contractor) NAME:_________________________________________ PHONE:________________________________________ Upon arrival to perform the required services, the Contractor is required to check in with COTR/Biomedical Engineering section phone 307-778-7362. All technician time sheets shall be signed only by authorized personnel in Biomedical Engineering. All services are to be performed by qualified personnel and in the manner prescribed by the user's manual. Failure to provide detailed copies of all service tickets to COTR or designee after each service call may result in decrease on invoice payment. The Chief Biomedical Engineering Technician or his designee is hereby delegated as Contracting Officer Technical Representative (COTR) for this contract and is responsible for providing any technical guidance required during performance of this contract. Telephone number for COTR is 307-778-7362. (COTR) is Ronald Dulemba Biomed Chief or his delegate. Work required in performance of this contract shall be performed during the VA Medical Center's regular administrative working hours, Monday thru Friday, 8 am to 5pm excluding Government holidays. Contractor may respond after these hours (only) if previously scheduled with COTR and at no additional cost to the government. QUALIFICATIONS OF CONTRACTOR AND AVAILABILTY OF PARTS Contractor must be established in the business of technical medical/ADP equipment repair and maintain or have available an inventory of repair/replacement parts, such that equipment down-time is limited to no longer than three (3) calendar days including allowable response time as specified elsewhere in this contract. All repair/replacement parts shall be at or above original manufacturer specifications. Contractor shall provide with written certification that all replacement parts are in compliance with Good Manufacturing Practices (GPM) as specified by FDA. Upon request by the Contracting Officer, the Contractor will provide a listing by description, part number and quantity of all replacement parts applicable to the equipment covered by this contract maintained by the Contractor in his/her inventory and/or a statement certified and signed by the Contractor that such parts as may be necessary for compliance with the terms of this contract are readily available to the Contractor within the timeframe allowed by the Contractor. All technicians performing services under this contract shall be fully factory trained and have hands on experience working on the Steris equipment mentioned in this contract. Contractor shall provide written proof of technicians training and experience. A list of references with telephone numbers (3 needed) to show experience on this type of equipment. QUALIFICATIONS: Quotes will be considered only from contractors who are regularly established in the business called for and who are financially responsible and have the supplies, necessary equipment, and trained personnel to furnish the services called for in this solicitation. SUBCONTRACTING: Subcontracting the services required under this contact WILL NOT BE ALLOWED without prior written approval from the Contracting Officer. n thereof.
